目的:利用经颅彩色多普勒(TCC)技术研究偏头痛间歇期颅内外动脉及肢体末梢动脉的血流动力学特点.方法:利用美国产Acusonl28XP/10型电脑彩超诊断仪对35例偏头痛间歇期患者及21例正常人大脑中动脉(MCA)、基底动脉(BA)、颞浅动脉深支(TSA)、食指桡侧动脉(RIA)的最大峰值流速(Vmax)、平均流速(v)、搏动指数(PI)、阻力指数(RI)进行观察分析.结果:①35例患者中,双侧大脑中动脉流速不对称者18例,占51%,双侧MCA流速对称而均升高者8例,占23%,双侧对称而均降低者2例,占6%,双侧对称而均正常者5例,占14%;②54%的患者MCA表现为一侧或双侧血流速度升高,PI指数下降(P<0.05);③BA与MCA表现为相同的变化趋势,但差异不明显;④TSA与对照组无明显变化(P>0.05);⑤80%的患者RIA表现为血流速度下降,搏动指数上升(P<0.05).结论:偏头痛间歇期颅内血管表现为流速上升,阻力下降,外周末梢循环表现为流速下降,阻力上升.
Objective: To study the hemodynamic characteristics of intracranial and extracranial arteries in patients with intermittent migraine by transcranial color Doppler (TCC) technique. Methods: Acusonl28XP / 10 computerized color Doppler ultrasound scanner was used to detect the changes of middle cerebral artery (MCA), basilar artery (BA), deep temporal branch of the superficial artery (TSA) The maximal peak velocity (Vmax), average velocity (V), pulsatility index (PI) and resistance index (RI) of RIA were observed and analyzed. RESULTS: Among the 35 patients, 18 patients (51%) had bilateral asymmetry of middle cerebral artery velocity and 8 patients (23%) both had symmetrical bilateral MCA flow velocity increase. Two patients were symmetrically reduced, Accounting for 6%, bilateral symmetry and were normal in 5 cases, accounting for 14%; ② 54% of patients showed one or both sides of the MCA blood flow velocity increased, PI index decreased (P <0.05); ③BA and MCA (P> 0.05); ⑤80% of the patients with RIA showed decreased blood flow velocity, pulsatility index increased (P <0.05). Conclusion: The intracranial blood vessels of intermittent migraine headache show the increase of flow velocity and decrease of resistance, and the peripheral circulation performance of the migraine is decreased and the resistance increases.
