探讨脉冲振荡法 (IOS)的测定指标 ,判断肺结核患者气管阻力和肺顺应性变化的临床意义。方法 :测定分析肺结核患者轻度组 49例 ,重度组 41例 ,肺心病组 19例和健康人 13例的各项 IOS指标和肺通气功能指标。结果 :肺结核各组随着病情加重 ,呼吸总阻抗 (Zrs)、总气管阻力 (R5 )、中心气管阻力 (R2 0 )、周围气管阻力 (R5 - R2 0 )及周边弹性阻力 (X5 )均逐渐增大。肺通气功能各指标则逐渐降低。与健康组比 ,肺结核轻度组呼吸阻抗各指标无显著差异 (P>0 .0 5 ) ,重度组 Zrs、R5 和 R5 - R2 0 明显增大 (P<0 .0 5 ) ,肺心病组呼吸阻抗各指标均显著增大 (P<0 .0 1)。相关分析示 Zrs与VC、Zrs与 MVV显著负相关 ,X5 与 VC、X5 与 MVV显著正相关 ,R5 - R2 0 与 MMEF显著负相关。结论 :IOS能够较好地反映肺结核呼吸阻抗及其各组分变化 ,值得进一步探讨和完善
To explore the clinical significance of determining the indexes of pulsed oscillatory method (IOS) to determine the changes of tracheal resistance and lung compliance in patients with pulmonary tuberculosis. Methods: The indexes of IOS and pulmonary function of 49 patients with mild tuberculosis, 41 severe patients, 19 patients with pulmonary heart disease and 13 healthy individuals were analyzed. Results: Respiratory resistance (Zrs), total tracheal resistance (R5), central trachea resistance (R2 0), tracheal resistance (R5 - R2 0) and peripheral elastic resistance (X5) gradually increased with the progression of pulmonary tuberculosis. Increase. Lung ventilation function index gradually decreased. Compared with the healthy group, there was no significant difference in the indexes of respiratory resistance between the mild group and the severe group (P <0.05), Zrs, R5 and R5 - R2 0 in the severe group were significantly increased (P <0.05) The indexes of respiration impedance increased significantly (P <0.01). Correlation analysis showed that Zrs was negatively correlated with VC, Zrs and MVV, X5 was significantly and positively correlated with VC, X5 and MVV, and R5 - R2 0 was negatively correlated with MMEF. Conclusion: IOS can better reflect the respiratory impedance of pulmonary tuberculosis and its components changes, it is worth to further explore and improve
