目的:评价闭合式平坦部玻璃体切除术对角膜内皮的影响。方法:采用TopconSP2000电脑角膜内皮显微镜对临床82例84眼闭合式平坦部玻璃体切除的病人行手术前、手术后角膜厚度(CT)和角膜内皮细胞密度(ECD)的动态测定,摄影观察细胞形态。辅以裂隙灯检查。利用病人健眼及患眼手术前后自身对照,结果行统计学处理。结果:各组CT术后均增加(P<0.001),呈可逆性变化,随时间延长,可逐渐恢复。ECD有晶体限组无明显变化(P>0.5),其余各观察组均有降低,尤其硅油填充组明显(P<0.001)。在尚未出现ECD减少的病例中,部分可发现有角膜内皮细胞形态的明显变化。结论:闭合式平坦部玻璃体切除术,可引起可逆性CT增加,无晶体眼ECD可显著减少,尤其是硅油眼,晶体的存在可降低手术对角膜内皮细胞的影响。
Objective: To evaluate the effect of closed flat vitrectomy on corneal endothelium. Methods: The topconSP2000 corneal endothelium microscope was used to measure the corneal thickness (CT) and corneal endothelial cell density (ECD) before and after surgery in 82 cases of 84 cases of closed vitreoretinal surgery. Supplemented by slit lamp inspection. Use of patients with eye health and eye surgery before and after their own control, the results of statistical analysis. Results: The CT in all groups increased (P <0.001), reversibly changed, and gradually recovered with time. There was no significant change in the crystal volume of ECD group (P> 0.5), and the rest of the observation groups decreased, especially in the silicone oil-filled group (P <0.001). In cases where no reduction in ECD has occurred, a significant change in the morphology of corneal endothelial cells can be found in some cases. Conclusion: The closed flat part vitrectomy can cause reversible CT increase, aphakia ECD can be significantly reduced, especially silicone oil eyes, the presence of crystals can reduce the impact of surgery on corneal endothelial cells.
