目的探讨凝血功能及血浆蛋白C水平在脓毒症休克中的预测价值。方法选取泉州市第一医院2015年2月—2016年3月收治的脓毒症休克患者50例为研究组,另选取健康体检者50例为对照组。比较两组受试者临床资料[呼吸频率(R)、脉搏(P)、平均动脉压(MAP)、白细胞计数(WBC)、体温(T)、谷氨酸氨基转移酶(ALT)]、凝血功能[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、D-D二聚体]及血浆蛋白C水平。结果两组P、R、MAP、WBC、T、ALT比较,差异无统计学意义(P>0.05)。研究组PT、TT、APTT、D-D二聚体高于对照组,血浆蛋白C水平低于对照组(P<0.05)。研究组中存活组PT、TT、APTT、D-D二聚体、血浆蛋白C水平高于死亡组(P<0.05)。结论凝血功能及血浆蛋白C水平可较好的预测脓毒症休克患者病情发展、严重程度及预后。
Objective To investigate the predictive value of coagulation and plasma protein C in septic shock. Methods Fifty patients with septic shock admitted to the First Hospital of Quanzhou from February 2015 to March 2016 were selected as the study group, and another 50 healthy subjects were selected as the control group. The clinical data of two groups were compared (respiratory rate, pulse, MAP, WBC, T, ALT) Function, prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), DD dimer and plasma protein C levels. Results There were no significant differences in P, R, MAP, WBC, T and ALT between the two groups (P> 0.05). The PT, TT, APTT and D-D dimers in the study group were higher than those in the control group, while the levels of plasma protein C in the study group were lower than those in the control group (P <0.05). Survival group PT, TT, APTT, D-D dimer, plasma protein C levels in the study group were higher than those in the death group (P <0.05). Conclusions Coagulation and plasma protein C level can predict the progression, severity and prognosis of patients with sepsis.
