目的探讨超声弹性成像评分法联合乳腺影像学报告及数据系统(BI-RADS-US)分类对乳腺肿块良、恶性病变的诊断价值。方法选择80例乳腺肿块患者,病理诊断为良性病变患者45例,恶性病变患者35例,所有患者均接受常规超声检查以及超声弹性成像,比较超声弹性成像评分法、BI-RADS-US分类法对乳腺肿块良、恶性病变的鉴别诊断价值,探讨两者联合诊断的价值。结果 80例患者共发现良性病灶70个,恶性病灶42个,恶性病灶超声弹性成像评分显著高于良性病灶,差异有统计学意义(χ~2=14.429,P<0.05);恶性病灶BI-RADS-US分类显著高于良性病灶,差异有统计学意义(χ~2=15.512,P<0.05);常规超声诊断敏感度仅为57.14%,超声弹性成像评分法与BI-RADS-US单独诊断的敏感度分别为83.33%和85.71%,两者联合诊断敏感度提高到95.24%,显著高于常规超声诊断,差异有统计学意义(χ~2=18.809,P<0.05),且Kappa值为0.751,一致性最好。结论超声弹性成像评分法联合BI-RADS-US分类可以提高鉴别诊断乳腺肿块良、恶性病变的敏感度,从而对病灶做出综合评价,提高诊断准确率。
Objective To investigate the diagnostic value of ultrasound elastography scoring combined with Breast Imaging Reporting and Data System (BI-RADS-US) in the diagnosis of benign and malignant breast lesions. Methods Eighty cases of breast masses were selected. Among them, 45 cases were pathologically diagnosed as benign lesions and 35 cases were malignant lesions. All patients underwent routine ultrasonography and ultrasound elastography.
