目的检测端粒酶逆转录酶(human telomerase reverse transcrip tase,hTERT)在膀胱癌患者外周血中的表达,探讨hTERT与尿路上皮细胞癌TNM分期的关系及对预后评估的意义。方法通过检测患者外周抗凝血RNA提取、反转录及荧光定量PCR扩增定量测定hTERT,用SPSS13.0软件进行单因素方差分析hTERT值。结果 46例膀胱肿瘤患者外周血中hTERT基因表达与是否浸润肌层密切相关(P<0.05)。非肌层浸润性膀胱肿瘤复发评分越高,外周血hTERT mRNA越高,但差异无显著性(P>0.05)。非肌层浸润性膀胱肿瘤进展评分越高,外周血hTERT mRNA越高,并且差异有统计学意义(P<0.05)。结论外周血hTERT mRNA可作为检测膀胱癌患者微转移的分子标志,hTERT可为手术治疗膀胱癌术式选择提供依据及判断预后。
Objective To detect the expression of human telomerase reverse transcriptase (hTERT) in the peripheral blood of patients with bladder cancer and to explore the relationship between hTERT and the TNM stage of urothelial carcinoma and its prognostic significance. Methods The peripheral blood of patients with anticoagulant RNA extraction, reverse transcription and quantitative PCR amplification of hTERT, using SPSS13.0 software for single-factor analysis of variance hTERT value. Results The expression of hTERT gene in peripheral blood of 46 patients with bladder cancer was closely related to infiltration of muscular layer (P <0.05). The higher the recurrence score of non-muscle invasive bladder tumor, the higher the peripheral blood hTERT mRNA, but no significant difference (P> 0.05). The higher the non-muscular invasive bladder tumor progression score, the higher the peripheral blood hTERT mRNA, and the difference was statistically significant (P <0.05). Conclusion hTERT mRNA in peripheral blood can be used as a molecular marker for detecting micrometastasis in patients with bladder cancer. HTERT can provide the basis for the surgical treatment of bladder cancer and determine the prognosis.
