目的:以富含SPA的金黄色葡萄球菌与鲍曼不动杆菌的多抗血清结合,制备特异的协同凝集试剂,探索其在鲍曼不动杆菌快速检测中的初步应用。方法:鲍曼不动杆菌免疫日本大耳白兔制备多抗血清并经吸收处理。富含SPA的金黄色葡萄球菌(No 1800株)经甲醛灭活后,与该多抗血清结合,制备协同凝集试剂,用各种临床分离菌株尤其是其它种类的不动杆菌与自制的协同凝集试剂反应,对该试剂的特异性和准确性进行评价。对临床新分离菌株,以本法和生物梅里埃公司的API 20E生化鉴定板对比,并结合细菌药敏结果判定自制协同凝集试剂的快速诊断效果。结果:鲍曼不动杆菌免疫日本大耳白兔获得了高质量的多抗血清,特异性好,凝集效价达1:320;抗体致敏的金黄色葡萄球菌,作为协同凝集试剂,能与待测鲍曼不动杆菌产生明显的凝集现象,灵敏度较血清凝集试验高16倍,而对其它种类的不动杆菌以及其它种属的细菌均无凝集现象。与传统的生化鉴定相比,协同凝集试验可提前1~2天得到细菌鉴定结果,结果准确性良好。结论:以鲍曼不动杆菌的多抗血清致敏富含SPA的金黄色葡萄球菌作为协同凝集试剂,对鲍曼不动杆菌具有特异、灵敏、快速的检测效果,可以作为常规诊断方法的有益补充。
OBJECTIVE: To establish a specific synergistic coagulation reagent by combining SPA-rich Staphylococcus aureus with multi-antiserum of Acinetobacter baumannii and to explore its preliminary application in the rapid detection of Acinetobacter baumannii. Methods: Acinetobacter baumannii was used to immunize Japanese white rabbits to prepare multiple antiserums and absorb them. The SPA-enriched Staphylococcus aureus (No. 1800 strain) is inactivated by formaldehyde and then combined with the polyclonal antiserum to prepare a cohesive coagulant, which is co-agglutinated with various clinically isolated strains, especially other Acinetobacter species, The reagents were reacted and the specificity and accuracy of the reagents were evaluated. The clinical isolates were compared with the API 20E biochemical identification kit of BioMerieux and the rapid diagnosis results of the self-made co-agglutination reagent in combination with the bacterial susceptibility results. Results: Acinetobacter baumannii immunized Japanese white rabbits to obtain high-quality multi-antiserum with good specificity and agglutination titer of 1: 320. Antibody-sensitized Staphylococcus aureus, as a cohesive agglutination reagent, Acinetobacter baumannii to produce obvious agglutination phenomenon, the sensitivity of 16 times higher than the serum agglutination test, and other species of Acinetobacter and other species of bacteria are not agglutination phenomenon. Compared with the traditional biochemical identification, the co-agglutination test can be 1 to 2 days ahead of time to get the results of bacterial identification, the result is good. CONCLUSIONS: Acinetobacter baumannii multi-antiserum sensitizes SPA-rich Staphylococcus aureus as a cohesive agglutination reagent, which has a specific, sensitive and rapid detection effect on Acinetobacter baumannii and can be used as a routine diagnostic method supplement.
