目的探讨慢性心力衰竭患者营养风险筛查评分与呼吸道感染的关系,进一步探讨引起呼吸道感染的病原菌,总结临床治疗经验,根据患者自身情况有针对性地对患者进行更好的治疗。方法选取2012年1月至2014年1月期间三门峡市直机关医院接收治疗的108例慢性心力衰患者,将108例慢性心力衰竭患者分为观察组和对照组,每组54例。观察组为慢性心力衰竭伴呼吸道感染患者,对照组为慢性心力衰竭患者。对比两组患者的营养风险和病毒分布、构成情况。结果 108例患者共送检痰标本129次,排除两组患者痰培养过程中的标本污染情况,共培养出病原菌198株,其中革兰阴性菌91株,占45.96%,革兰阳性菌63株,占31.31%,真菌44株,占22.73%。观察组营养情况显著优于对照组,差异有统计学意义(P<0.05)。结论革兰阴性菌是慢性心力衰竭伴呼吸道感染患者的主要病原菌,且慢性心力衰竭伴呼吸道感染患者营养不良的发生率显著高于慢性心力衰竭患者。
Objective To explore the relationship between nutritional risk screening score and respiratory tract infection in patients with chronic heart failure, to further explore the pathogenic bacteria causing respiratory tract infection, summarize the experience of clinical treatment, and to provide targeted treatment to the patients according to their own circumstances. Methods From January 2012 to January 2014, 108 cases of chronic heart failure admitted to the Straightening Hospital of Sanmenxia City were enrolled. 108 cases of chronic heart failure were divided into observation group and control group, with 54 cases in each group. The observation group was chronic heart failure patients with respiratory tract infection, the control group was chronic heart failure patients. The nutritional risk and the distribution of the virus in two groups were compared and the situation was formed. Results A total of 108 sputum samples were sent out for 129 times to exclude the contamination of sputum in the two groups. A total of 198 pathogens were isolated, of which 91 were Gram-negative bacteria (45.96%) and 63 were Gram-positive bacteria , Accounting for 31.31%, 44 fungi, accounting for 22.73%. The nutritional status of the observation group was significantly better than that of the control group, the difference was statistically significant (P <0.05). Conclusions Gram-negative bacteria are the main pathogens in patients with chronic heart failure and respiratory tract infection, and the incidence of malnutrition in patients with chronic heart failure and respiratory tract infection is significantly higher than that in patients with chronic heart failure.
