选择性5-HT再摄取抑制剂(SSRIs)是一类新药物,这类药物的副反应与相互作用尚未完全阐明。1例54岁男子,于冠状动脉搭桥术后4年发生心绞痛,而用美托洛尔100mg每天1次治疗,心绞痛改善,休息时心率为64次/min。1月后因抑郁症用氟西汀20mg每天1次治疗,2天后出现嗜睡并有心动过缓(心率36次/min),停用氟西汀治疗。其后5天心率恢复正常,因此疑为该药与美托洛尔的相互作用,因而停用美托洛尔,改用水溶性β-阻滞剂索他洛尔80 mg,每天2次。1周后再用氟西汀治疗,未再出现心动过缓。
Selective 5-HT reuptake inhibitors (SSRIs) are a new class of drugs whose side effects and interactions have not yet been fully elucidated. A 54-year-old man developed angina at 4 years after coronary artery bypass surgery. Treatment with metoprolol 100 mg once daily resulted in improvement in angina, with a resting heart rate of 64 beats / min. 1 month after treatment with fluoxetine 20mg once daily for depression, drowsiness and bradycardia 2 days later (heart rate 36 beats / min), the withdrawal of fluoxetine treatment. The next 5 days the heart rate returned to normal, so the drug is suspected of interaction with metoprolol, thus disabling metoprolol, use water-soluble beta blockers sotalol 80 mg twice daily. After 1 week and then treated with fluoxetine, no recurrence of bradycardia.
