目的 探讨不同类型胃息肉与幽门螺杆菌 (Hp)感染以及与胃粘膜炎症、萎缩 和肠上皮化生的相关性。方法 对 2 203例胃粘膜活检发现的 278例胃息肉 (检出率 12.6 % )进行组织学分类,并对胃窦粘膜活检组织同时行 Hp检测及粘膜炎症、萎缩和肠上皮化 生的观察,比较不同类型胃息肉的 Hp感染率及其与癌相关病变的关系。结果 278例息肉中 ,组织学分类以小凹上皮增生型息肉为多见,有 130例,占息肉总数的 46 8%,胃体腺 型增生性息肉 67例,占 24 1%,炎性息肉和腺瘤性息肉较少见,分别为 55例 (19.8% )和 26例 (9.4% )。约 53 9%的胃息肉患者存在 Hp感染,其中以小凹上皮增生型息肉 感染率最高,达 73.1%。这型息肉常伴有胃粘膜的活动性炎症,且粘膜萎缩和肠上皮化生 的发生率均接近腺瘤性病变。胃体腺型增生性息肉, Hp感染率、活动性炎症及萎缩、肠上 皮化生等发生率均较低,提示这两种息肉在组织发生和病理生物学形态上存在差异。结论 小凹上皮增生型息肉的发生可能与 Hp感染有关,且常伴有明显的活动性炎症以及粘膜萎缩 和肠上皮化生,因此可能与胃癌的发生有潜在的关系。
Objective To investigate the relationship between different types of gastric polyp and Helicobacter pylori (Hp) infection, and inflammation, atrophy, and intestinal metaplasia in gastric mucosa. Methods A total of 278 cases of gastric polyps (detection rate of 12.6%) found in gastric mucosa biopsy were classified by histology. Gastric mucosal biopsies were also tested for Hp and mucosal inflammation, atrophy, and intestinal metaplasia. To compare the Hp infection rate of different types of gastric polyps and their relationship with cancer-related lesions. Results Of the 278 polyps, histological classifications were epithelial hyperplastic polyps, which were more common in 130 cases, accounting for 46.8% of the total number of polyps, and 67 cases of gastric glandular hyperplastic polyps (24.1%). Polyps and adenomatous polyps are less common, with 55 cases (19.8%) and 26 cases (9.4%), respectively. About 53.9% of gastric polyp patients had H.pylori infection, of which the epithelial hyperplastic polyp infection rate was the highest, reaching 73.1%. This type of polyp is often accompanied by active inflammation of the gastric mucosa, and the incidence of mucosal atrophy and intestinal metaplasia are similar to adenomatous lesions. The incidence of glandular hyperplastic polyps, Hp infection rate, active inflammation and atrophy, and intestinal metaplasia were all lower, suggesting that there are differences in histogenesis and pathobiology between the two types of polyps. Conclusion The occurrence of small pit epithelial hyperplasia polyps may be related to Hp infection, and often accompanied by active inflammation, mucosal atrophy and intestinal metaplasia, which may have a potential relationship with the occurrence of gastric cancer.
