目的:探讨采用螺旋CT三维重建技术在颌面部复杂性骨折手术治疗过程中的作用与价值。方法:选取2013年1月—2014年9月就诊的7例复杂性颌面部外伤患者作为研究对象,采用高分辨率CT(HRCT)对患者面部进行扫描,然后容积再现法(VR)进行三维成像,并据此制订初步的手术治疗方案。结果:所有患者均呈现颌面部复杂性骨折,上颌骨7例,其中伴眼眶、鼻骨及鼻窦骨折7例,伴上颌窦前、后壁塌陷及眶下、外壁、鼻骨、蝶骨、筛骨等多处骨折;颧弓骨折5例。所有患者均行骨折切开复位加内固定术,术后均获得满意疗效。结论:利用螺旋CT三维重建技术,可以清晰地显示颌面部骨折的部位、移位程度、骨折线的走行,对于手术方案的制订和实施具有重要的指导意义。
Objective: To explore the role and value of spiral CT three-dimensional reconstruction in the surgical treatment of maxillofacial complex fractures. Methods: Seven patients with complicated maxillofacial trauma admitted to our hospital from January 2013 to September 2014 were selected as the study objects. The face of the patient was scanned by high resolution CT (HRCT), and then volumetric rendering (VR) Imaging, and the development of a preliminary surgical treatment plan accordingly. Results: All the patients presented maxillofacial complex fractures in 7 cases, including 7 cases of orbital, nasal and sinonasal fractures with anterior and posterior wall collapse of the maxillary sinus, infraorbital wall, nasal bone, sphenoid bone, ethmoid bone And other fractures; zygomatic arch fracture in 5 cases. All patients underwent open reduction and internal fixation of fracture, and satisfactory results were obtained after operation. Conclusion: The three-dimensional reconstruction of spiral CT can clearly show the location of maxillofacial fractures, the degree of displacement and the fracture line. It has important guiding significance for the formulation and implementation of surgical plans.
