目的:探讨快测C反应蛋白(CRP)水平与呼吸道感染的关系。方法:回顾性分析2016年1月至2016年6月佛山市第一人民医院脑科康复医院收治的呼吸道感染患儿400例,所有患儿均给予快测CRP检测、血常规和实验室检查,以实验室检查为对照组,依据病原菌分为细菌组(250例)和非细菌组(150例),采用ROC曲线评估快测CRP检测、血常规鉴别诊断呼吸道感染的效能,统计分析所有患儿CRP、白细胞计数(WBC)水平及不同方法鉴别诊断呼吸道感染的敏感性、特异性和准确性情况。结果:细菌组患儿CRP、WBC水平明显高于非细菌组,差异具有统计学意义(P<0.05);ROC曲线结果显示,快测CRP检测鉴别诊断呼吸道感染的敏感度、特异度和准确度分别为92.40%、88.00%、90.75%,WBC敏感度、特异度和准确度分别为85.20%、80.00%、83.25%,前者明显高于后者,两组敏感度和准确度比较差异具有统计学意义(P<0.05),但特异度比较差异均无统计学意义(P>0.05)。结论:CRP、WBC水平变化与细菌性呼吸道感染的发生有关,其中快测CRP较WBC更灵敏,可作为优先选择的诊断方法之一。
Objective: To investigate the relationship between fasting C-reactive protein (CRP) level and respiratory infection. Methods: A retrospective analysis of 400 children with respiratory tract infection admitted to the First Affiliated Hospital of Foshan City from January 2016 to June 2016 was carried out. All children were given CRP, blood tests and laboratory tests. According to the laboratory test, the patients were divided into bacteria group (250 cases) and non-bacteria group (150 cases) according to the pathogenic bacteria. The ROC curve was used to evaluate the rapid detection of CRP and the blood routine to differentiate the respiratory infection. All the children CRP, WBC levels and the sensitivity, specificity and accuracy of differential diagnosis of respiratory infections. Results: The levels of CRP and WBC in the bacterial group were significantly higher than those in the non-bacterial group (P <0.05). The ROC curve showed that the sensitivity, specificity and accuracy of the rapid detection of CRP in the diagnosis of respiratory infection The sensitivity, specificity and accuracy of WBC were 85.20%, 80.00% and 83.25% respectively, the former was significantly higher than the latter, the differences of sensitivity and accuracy between the two groups were statistically significant (92.40%, 88.00%, 90.75% (P <0.05), but there was no significant difference in the specificity (P> 0.05). Conclusion: The changes of CRP and WBC levels are related to the occurrence of bacterial respiratory infections. The rapid detection of CRP is more sensitive than that of WBC, which may be one of the preferred diagnostic methods.
