PCR与膜芯片分型法检测女性生殖道HPV结果比较分析

目的分析荧光探针PCR与膜芯片分型技术两种方法检测女性生殖道人乳头状瘤病毒(HPV)的结果,对两种方法进行临床应用评价。方法随机收取2015年01月~11月来本院门诊就诊的女性患者1062例,利用荧光探针PCR定性检测14种常见的高危型HPV病毒(包括HPV16,18,31,33,35,39,45,51,52,56,58,59,68,66),并对其中的HPV16和HPV18型进行分型检测,同时用膜芯片分型技术检测23种HPV型,包括上述14种常见高危HPV外,还包括4种高危型(HPV53,73,82,83)和5种低危型HPV病毒(HPV6,11,42,43,81),并对两种方法检测的结果进行对比分析。结果荧光探针PCR定性检测HPV的阳性率为12.90%(137/1062),膜芯片分型技术检测HPV阳性率19.68%(209/1062),阳性率明显高于探针PCR法,两者阳性率之间差异有统计学意义(χ~2=15.627,P<0.05);但两种方法检测14种常见的高危型HPV(HPV16,18,31,33,35,39,45,51,52,56,58,59,68,66)病毒阳性率之间差异无统计学意义(χ~2=1.063,P>0.05),总符合率为91.15%(968/1062),阳性符合率为91.97%(126/137),阴性符合率为91.03%(842/925),两者检测结果具有较好的一致性。结论荧光探针PCR与膜芯片分型技术检测常见的14种高危型HPV的结果具有良好的一致性,阳性率之间差异无统计学意义,但膜芯片分型技术检测HPV的基因型数比荧光探针PCR要多,检出HPV阳性率要高,更适合女性大规模健康体检。 Objective To analyze the results of fluorescent probe PCR and membrane chip typing in detecting female genital tract human papillomavirus (HPV), and evaluate the clinical application of the two methods. Methods A total of 1062 female patients from our hospital from January to November in 2015 were enrolled in this study. 14 common high-risk HPV viruses (including HPV16, 18, 31, 33, 35, 39, 45,51,52,56,58,59,68,66), and the type of HPV16 and HPV18 type detection, and the use of membrane chip typing detection of 23 types of HPV, including the above-mentioned 14 kinds of common high-risk HPV Four high-risk HPV types (HPV53,73,82,83) and five low-risk HPV types (HPV6,11,42,43,81) were also included. The results of the two methods were compared. Results The positive rate of HPV detected by fluorescent probe PCR was 12.90% (137/1062). The positive rate of HPV detected by membrane chip technique was 19.68% (209/1062), the positive rate was significantly higher than that of probe PCR The difference between the two groups was statistically significant (χ ~ 2 = 15.627, P <0.05). However, the two methods were used to detect 14 kinds of common high-risk HPV (HPV16,18,31,33,35,39,45,51,52 , 56,58,59,68,66). There was no significant difference between the two groups (χ ~ 2 = 1.063, P> 0.05). The overall coincidence rate was 91.15% (968/1062). The positive coincidence rate was 91.97 % (126/137), the negative coincidence rate was 91.03% (842/925), the two test results have good consistency. Conclusion The results of fluorescent probe PCR and membrane chip typing in detecting 14 common high-risk HPVs showed good consistency. There was no significant difference between the positive rate and the fluorescent probe PCR, but the genotypic ratio Fluorescent PCR should be more, the positive rate of HPV detected is higher, more suitable for women’s large-scale health examination.
