目的探讨血糖波动对2型糖尿病合并脑卒中患者运动功能及日常生活活动能力的影响。方法选取2型糖尿病合并脑卒中患者60例,根据平均血糖波动幅度(MAGE)将患者分为低MAGE组(L组)35例、高MAGE组(H组)25例。两组患者均给予综合康复治疗。对患者治疗前及治疗4周后采用Fugl-Meyer运动功能评定量表(FMA)和Barthel指数评定表(BI)评定患者的运动功能和日常生活活动能力。结果 L组运动功能及日常生活活动能力优于H组(P<0.05);FMA与平均血糖(MBG)、MAGE呈负相关(r=-0.42、-0.58,P<0.05);BI与MBG、MAGE呈负相关(r=-0.44、-0.53,P<0.05)。结论血糖波动影响2型糖尿病合并脑卒中患者运动功能及日常生活活动能力,血糖波动大的患者运动功能及日常生活活动能力恢复较差。
Objective To investigate the effect of blood glucose fluctuation on motor function and activities of daily living in type 2 diabetic patients with stroke. Methods Sixty patients with type 2 diabetes mellitus and stroke were enrolled. 35 patients were divided into low MAGE group (L group) and 25 high MAGE group (H group) according to mean blood glucose fluctuation range (MAGE). Two groups of patients were given comprehensive rehabilitation. Before and after treatment for 4 weeks, Fugl-Meyer Motor Function Rating Scale (FMA) and Barthel Index Rating Scale (BI) were used to assess the motor function and activities of daily living. Results The motor function and activities of daily living in group L were better than those in group H (P <0.05). There was a negative correlation between FMA and MBG and MAGE (r = -0.42, -0.58, P <0.05) MAGE was negatively correlated (r = -0.44, -0.53, P <0.05). Conclusion The fluctuation of blood glucose affects the motor function and activities of daily living in patients with type 2 diabetes mellitus complicated with stroke. The patients with large blood glucose fluctuation have poor recovery of motor function and activities of daily living.
