[目的]评价新型农村合作医疗(新农合)定点医疗机构的服务质量,探讨存在的问题与解决办法,为有关卫生主管部门改进各级定点医疗机构服务质量提供科学依据。[方法]2005年,在云南省20个新农合试点县中选择龙陵县,对参合农民进行问卷调查。[结果]调查191人,对各级定点医疗机构医疗技术水平、服务态度、收费、总体满意度和卫生资源配置情况的评价分布的差异均有统计学意义(P<0.01);非条件Logistic回归分析表明,调查对象喜欢选择村卫生室就诊,其主要原因是医生服务态度好,可信度高,就医方便。[结论]各级定点医疗机构服务质量一般,应提高乡村卫生机构的服务条件,改善县级医疗机构的服务态度,严格控制农村医药费用的不合理增长。
[Objective] To evaluate the service quality of the new-type rural cooperative medical care (NCMS) fixed-point medical institutions, explore the existing problems and solutions, and provide scientific basis for the relevant health authorities to improve the service quality of fixed-point medical institutions at all levels. [Methods] In 2005, Longling County was selected among 20 new rural cooperative pilot counties in Yunnan Province and a questionnaire survey was conducted among participating farmers. [Results] There were 191 surveys, and there were statistically significant differences in the distribution of medical technology, service attitudes, fees, overall satisfaction, and health resource allocations among fixed-point medical institutions at all levels (P<0.01); unconditional logistic regression. The analysis shows that the respondents like to choose the village clinic for treatment. The main reason is that doctors have good service attitude, high credibility, and convenient medical treatment. [Conclusion] The service quality of fixed-point medical institutions at all levels is normal. The service conditions of rural health institutions should be improved, the service attitude of county-level medical institutions should be improved, and the unreasonable growth of rural medical expenses must be strictly controlled.
