目的探讨磁共振扩散加权成像的诊断方法以及早期前列腺癌的鉴别诊断方法。方法选取平舆县人民医院2012年5月至2014年4月诊断的77例患者,其中35例良性前列腺增生患者为良性组,32例前列腺癌患者为腺癌组,所有患者均行常规的T2WI、DWI以及动态增强T1WI扫描。观察显示结果,并设定兴趣区,测量并统计分析其ADC值。结果在腺癌组中,恶性组织的平均ADC值明显低于正常组织,差异有统计学意义(P<0.05);在良性组中,增生组织中央区的平均ADC值与外周带相比,差异未见统计学意义(P>0.05);两组间比较,腺癌组中正常组织的平均ADC值与良性组相比,差异未见统计学意义(P>0.05)。结论磁共振扩散加权成像检查和ADC值有助于早期前列腺癌的发现诊断,尤其对于前列腺中央区和前列腺尖部发生的前列腺癌诊断更为明显,且临床诊断中结合T2WI能有效提高早期前列腺癌的诊断检出率。
Objective To investigate the diagnostic methods of diffusion-weighted imaging and early differential diagnosis of prostate cancer. Methods 77 patients diagnosed in Pingyu County People’s Hospital from May 2012 to April 2014 were selected, in which 35 cases of benign prostatic hyperplasia were benign group and 32 cases of prostate cancer were adenocarcinoma group. All patients underwent conventional T2WI , DWI, and dynamically enhanced T1WI scans. Observe the results show, and set the area of interest, measurement and statistical analysis of ADC value. Results In adenocarcinoma group, the average ADC value of malignant tissue was significantly lower than that of normal tissue (P <0.05). In benign group, the average ADC value of central zone of proliferative tissue was significantly lower than that of peripheral zone There was no significant difference between the two groups (P> 0.05). There was no significant difference between the two groups in the average ADC value of normal tissue in benign adenocarcinoma group and benign group (P> 0.05). Conclusion MRI diffusion weighted imaging and ADC values are helpful for the diagnosis of early-stage prostate cancer, especially for the diagnosis of prostate cancer in the central region of the prostate and prostatic apex, and the combination of T2WI and clinical diagnosis can effectively improve the early-stage prostate cancer The diagnostic detection rate.
