目的探讨硝普钠与多巴胺联合呋塞米治疗顽固性心力衰竭患者的临床效果及安全性。方法选取2014年1月至2015年1月本溪市中心医院治疗的71例顽固性心力衰竭患者作为研究对象,按随机数字表法将其分为观察组(36例)和对照组(35例)。对照组患者在常规内科治疗基础上给予硝普钠与多巴胺治疗,观察组患者在对照组基础上静脉滴注呋塞米注射液,比较两组患者的临床治疗效果、心功能变化情况及不良反应发生情况。结果观察组患者治疗的总有效率明显高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者的心脏左心室射血分数(LVEF)、左心室短轴缩短率(LVFS)均明显高于治疗前,左心室舒张末期内径(LVEDd)明显低于治疗前,但观察组患者的LVEF明显高于对照组,差异均有统计学意义(均P<0.05);两组患者治疗过程中均无严重不良反应发生。结论硝普钠与多巴胺联合呋塞米治疗顽固性心力衰竭临床效果明显,可有效改善患者的心脏功能,且不良反应少,使用安全可靠。
Objective To investigate the clinical effect and safety of sodium nitroprusside and dopamine combined with furosemide in treatment of patients with refractory heart failure. Methods 71 patients with refractory heart failure treated in Benxi Central Hospital from January 2014 to January 2015 were enrolled in this study. Patients were divided into observation group (36 cases) and control group (35 cases) by random number table method. . Patients in the control group were treated with sodium nitroprusside and dopamine on the basis of routine medical treatment. The observation group received intravenous furosemide injection on the basis of the control group, and the clinical effects, cardiac function changes and adverse reactions in the two groups were compared What happened? Results The total effective rate of observation group was significantly higher than that of control group (P <0.05). After treatment, the left ventricular ejection fraction (LVEF), shortening rate of left ventricular short axis LVFS) were significantly higher than before treatment, the left ventricular end diastolic diameter (LVEDd) was significantly lower than before treatment, but the observation group patients LVEF was significantly higher than the control group, the difference was statistically significant (P <0.05); two groups No serious adverse reactions occurred during the treatment of patients. Conclusion Sodium nitroprusside and dopamine combined with furosemide in the treatment of refractory heart failure have obvious clinical effects, which can effectively improve the cardiac function of patients with less adverse reactions and safe and reliable use.
