目的应用中西医结合方法防治自发性高血压大鼠(SHR)心室重构中心肌纤维化的病理改变。方法8周龄SHR 40只,随机分为4组,每组10只,并以8周龄SD大鼠10只作为对照组,共5组:正常组、模型组、治疗组(降压脉净颗粒剂+络活喜)、西药对照组(络活喜)、中药对照组(牛黄降压丸+络活喜)。药物:降压脉净颗粒剂(桑寄生、、决明子、丹参、生山楂等组成),溶成药液,含药量1.3 g/mL;络活喜加蒸馏水溶成药液,含药量0.2 g/L;牛黄降压丸加蒸馏水溶成药液,含药量19.5 g/L。3种药物剂量均10 mL/kg,每日灌胃,疗程20周。观察实验动物的心脏重量指数、心肌胶原容积分数(CVF)、血管周围胶原面积(PVCA),Ⅰ、Ⅲ型胶原的表达。电镜观察其病理改变。免疫组织化学法检测心肌组织基质金属蛋白酶-2(MMP-2)蛋白表达水平。结果心脏重量指数、CVF、PVCA:治疗组与模型组比较减少,差异呈显著性(P<0.01)。与西药对照组和中药对照组比较亦有减少;病理学改变:偏光镜下观察Ⅰ、Ⅲ型胶原的表达中治疗组的表达明显比模型组减少,与西药对照组和中药对照组比较也有减少趋势。结论SHR心脏重量指数及心肌间质及血管周围胶原纤维明显增生,心肌中胶原蛋白含量明显增加的机制成为导致心肌纤维化、心力衰竭的原因;中药降压脉净剂具有抑制自发性高血压大鼠心肌纤维化的作用,在干预和逆转心肌纤维化方面优于单纯西药络活喜对照组,也有优于中药对照组的趋势。
Objective To apply traditional Chinese and western medicine methods to prevent and treat the pathological changes of myocardial fibrosis in ventricular remodeling in spontaneously hypertensive rats (SHR). Methods Forty 8-week-old SHR were randomly divided into 4 groups with 10 rats in each group. Eight 8-week-old SD rats were used as the control group. Compared with western medicine control group and traditional Chinese medicine control group, the pathological changes: the expression of collagen type I and III in the treatment group was significantly lower than that in the model group, and it was also decreased compared with the western medicine control group and the traditional Chinese medicine control group. trend. Conclusion The heart weight index, myocardial interstitial and perivascular collagen fibers in SHR were significantly proliferated, and the mechanism of the increase of collagen content in myocardium became the cause of myocardial fibrosis and heart failure. Chinese medicine Jiangyajingjing can inhibit spontaneous hypertension. The effect of myocardial fibrosis in rats was superior to that of Western medicine Luoxixi control group in intervention and reversal of myocardial fibrosis, and it was also superior to Chinese medicine control group.
