目的观察接种卡介苗婴幼儿淋巴结强阳性反应的免疫状况。方法选取2008年1月~2015年8月我院接种的卡介苗后淋巴结强阳性反应的患儿80例,分为观察组与正常反应的患儿的对照组,各40例。分别收集各组全血及血清,用流式细胞仪检测抗凝全血中CD4+T、CD8+T及散射比浊法检测Ig M、Ig G、Ig A水平。结果观察组全血CD4+T、CD4/CD8低于对照组,差异有统计学意义(P<0.05),CD8+T细胞降低不明显,差异无统计学意义(P>0.05);观察组血清中Ig G的含量低于对照组,差异有统计学意义(P<0.05)。结论目前对卡介苗接种所引淋巴结强阳性反应,可能由于种种原因导致组织中巨噬细胞和致敏淋巴细胞迅速被激活,引起免疫反应过程,因此接种处附近淋巴结(常为腋下)出现巨噬细胞的集聚,大量吞噬卡介苗,形成以增生为主的变化,当机体抵抗力低下时可继发干酪样坏死,致机体细胞和体液免疫降低。
Objective To observe the immunological status of strong positive lymph nodes reaction in BCG vaccination. Methods Eighty children with BCG-positive post-BCG vaccination in our hospital from January 2008 to August 2015 were selected and divided into observation group and control group, with 40 cases in each group. The whole blood and serum of each group were collected, and the levels of CD4 + T, CD8 + T and Scattering turbidimetric assay were determined by flow cytometry. Results The levels of CD4 + T and CD4 / CD8 in the whole blood of the observation group were lower than those of the control group (P <0.05), but there was no significant difference in the CD8 + T cells (P> 0.05) The content of Ig G was lower in the control group, the difference was statistically significant (P <0.05). Conclusions Currently, the strong positive response to BCG vaccination leads to the rapid activation of macrophages and sensitized lymphocytes in tissues due to various reasons, resulting in an immune response. Therefore, macrophages appear in lymph nodes (often underarm) near the inoculation sites Aggregation of cells, a large number of phagocytosis BCG, the formation of hyperplasia-based changes, when the body resistance to low casein-like necrosis can be induced, resulting in decreased body cells and humoral immunity.
