目的探讨慢性丙型肝炎(CHC)患者和健康志愿者外周血CD4+CXCR5+PD-1+T淋巴细胞与血清球蛋白之间的相关性,明确外周血滤泡辅助性T淋巴细胞(Tfh)转录因子无刚毛鳞甲复合体同源物样蛋白(Ascl)2的表达在慢性HCV感染过程中的作用。方法选取2015年10月-2016年5月第四军医大学唐都医院收治的住院或门诊初治CHC患者46例,另选取实验室健康成年实验人员以及本院健康体检者32例作为健康对照组。应用流式细胞技术分析外周血中Tfh细胞在CD4+T淋巴细胞中所占比例及其亚群(CD4+CXCR5+PD-1+T淋巴细胞)变化,应用全自动生化分析仪检测血清球蛋白水平,并通过real-time PCR检测Tfh细胞中Ascl2 mRNA表达情况;分析各指标变化与HCV感染的相关性。计量资料组间比较采用独立样本t检验,相关性分析采用Pearson相关分析。结果 CHC患者外周血Tfh细胞与B淋巴细胞呈正相关(r=0.582 3,P=0.011 2),外周血B淋巴细胞与球蛋白呈正相关(r=0.450 9,P=0.031 6),Tfh细胞与球蛋白呈正相关(r=0.583 5,P=0.038 3);CHC患者外周血Tfh细胞中Ascl2mRNA表达显著高于健康对照组(1.019±0.666 vs 6.437±5.776,t=4.552,P=0.001 9)。结论 Tfh细胞与CHC患者血清中球蛋白的产生有一定的联系,转录因子Ascl2可能参与HCV感染过程中Tfh细胞的分化发育。
Objective To investigate the relationship between CD4 + CXCR5 + PD-1 + T lymphocytes and serum globulin in peripheral blood of patients with chronic hepatitis C (CHC) and healthy volunteers, and to clarify the relationship between peripheral blood follicular helper T lymphocytes (Tfh) Role of Transcription Factor in the Expression of Asymmetric Aspergill 2 Complex in the Chronic HCV Infection. Methods Forty-six CHC patients in hospital or outpatient department were enrolled in this study from October 2015 to May 2016 in Tangdu Hospital of Fourth Military Medical University. In addition, 32 healthy healthy adult subjects and 32 healthy subjects were selected as healthy control group . Flow cytometry was used to analyze the changes of Tfh cells in CD4 + T lymphocytes and their subsets (CD4 + CXCR5 + PD-1 + T lymphocytes). Serum globulin Level, and the expression of Ascl2 mRNA in Tfh cells was detected by real-time PCR. The correlation between the changes of each index and HCV infection was analyzed. Measurement data were compared between groups using independent samples t-test, correlation analysis using Pearson correlation analysis. Results There was a positive correlation between Tfh cells and B lymphocytes in peripheral blood of patients with CHC (r = 0.582 3, P = 0.011 2) and positive correlation between B lymphocytes and globulin (r = 0.450 9, P = 0.031 6) (R = 0.583 5, P = 0.038 3). The expression of Ascl2 mRNA in Tfh cells in CHC patients was significantly higher than that in healthy controls (1.019 ± 0.666 vs 6.437 ± 5.776, t = 4.552, P = 0.001 9). Conclusion Tfh cells have a certain relationship with the production of globulin in serum of CHC patients. The transcription factor Ascl2 may be involved in the differentiation and development of Tfh cells during HCV infection.
