目的:探讨脑梗死超负荷血糖与细胞间粘附分子-1(1CAM-1)的变化及其相关性。方法:脑梗死患者87例,根据血糖水平分为正常血糖组、应激性高血糖组和糖尿病组,分别于病程24小时、1周、2周与3周检测血清sICAM-1。结果:各组sICAM-1于24小时均有明显增高,1周时达高峰,后渐下降;应激性高血糖组、糖尿病组增高幅度更高;糖尿病组增高幅度最大、最持久。结论:超负荷血糖明显影响ICAM-1在脑梗死病理发生机制中的作用,控制脑梗死患者血糖及炎性反应为脑血管病的防治开辟新的途径。
Objective: To investigate the changes and their correlations of hyperglycemia and intercellular adhesion molecule-1 (1CAM-1) in cerebral infarction. Methods: Eighty-seven patients with cerebral infarction were divided into normal blood glucose group, stress hyperglycemia group and diabetic group according to blood glucose level. Serum sICAM-1 levels were measured at 24 hours, 1 week, 2 weeks and 3 weeks respectively. Results: The levels of sICAM-1 in each group were significantly increased at 24 hours, reaching the peak at 1 week and then gradually decreased. The stress hyperglycemia group and diabetes group had a higher increase rate, while the diabetes group had the largest and most prolonged increase rate. CONCLUSION: Hyperglycemia significantly affects the role of ICAM-1 in the pathogenesis of cerebral infarction. Controlling blood glucose and inflammatory response in patients with cerebral infarction opens up new avenues for the prevention and treatment of cerebrovascular diseases.
