目的:对乳腺叶状肿瘤的MRI表现分析。方法:对手术病理证实的12例乳腺叶状肿瘤MRI表现进行回顾性分析,包括平扫信号强度、形态学、增强病变内部强化特征的扫描、时间-信号强度曲线类型、ADC表现扩散系数值及DWI信号值。对正常乳腺组织ADC与乳腺叶状肿瘤PTs进行配对设计资料t的检验比较。结果:术后病理诊断12例乳腺叶状肿瘤PTs,其中良性7例,交界性4例,恶性1例。MRI上5例为椭圆形,7例为分叶状;4例欠清,8例边界清晰;12例乳腺叶状肿瘤PTs,MRI平扫TIWI过后,呈现低信号;STIR或T2WI脂肪抑制呈现高信号,其中3例肿瘤内见囊性极高不规则信号影,3例见低信号分隔;乳腺叶状肿瘤PTs在初期呈动态快速增强状态,初始期后TIC9例呈II型,3例呈I型;DWI检查11例,其表现均为高信号,正常乳腺组织ADC值与患者平均ADC值相比有显著差异,具有统计学意义(p<0.05)。结论:乳腺叶状肿瘤PTs的MRI表现特点无明显特征,因此为非特征性,需经过组织病理学检查确诊。
Objective: To analyze the MRI findings of breast phyllodes tumor. Methods: The MRI findings of 12 cases of breast phyllodes tumor confirmed by surgery and pathology were retrospectively analyzed. The results showed that the signal intensity, morphology, enhanced scan of internal lesion, type of time-signal intensity curve, ADC diffusion coefficient and DWI signal value. The normal breast tissue ADC and breast leaf tumor PTs paired design data t test comparison. Results: Postoperative pathological diagnosis of 12 cases of breast phyllodes tumor PTs, including benign in 7 cases, 4 cases of borderline, 1 case of malignant. 5 cases were oval in MRI, 7 cases were lobulated, 4 cases were poorly clear, and 8 cases had clear border. In 12 cases of breast phyllodes tumor, the signal intensity of PTs and MRI showed low signal after TIWI. The fat suppression of STIR or T2WI was high Among them, 3 cases showed highly cystic irregular signal in 3 cases, and low signal separation in 3 cases. PTs in mammary gland leaf tumor showed dynamic rapid enhancement in initial stage, and TIC9 cases showed type II in initial stage and 3 cases showed I Type; DWI examination in 11 cases, the performance is high signal, the ADC value of normal breast tissue with the average ADC value of patients were significantly different, with statistical significance (p <0.05). Conclusion: MRI features of PTs in breast phyllodes tumor have no obvious features, so they are non-characteristic and need to be confirmed by histopathological examination.
