目的探讨C-反应蛋白(CRP)、糖类抗原125(CA125)、心肌肌钙蛋白I(c Tn I)3项指标联合检测在慢性心力衰竭(CHF)诊断中的应用价值。方法择取2014年5月到2016年5月山东德州市中医院治疗的60例CHF患者(CHF患者组),选择同期在本院进行体检的健康受检者60例(健康对照组)。取受试者禁食后第2日清晨空腹静脉血,检测两组CRP、CA125、c Tn I的结果,并比较CHF不同心功能患者3项指标的差异。结果 CHF患者组CRP、CA125、c Tn I 3项指标测定值均明显高于健康对照组[CRP(mg/L):9.02±4.06比2.20±1.19,CA125(U/L):47.95±6.03比6.51±0.47,c Tn I(μg/L):0.49±0.04比0.02±0.01,P均<0.05],并且心功能Ⅲ~Ⅳ级CHF患者上述3项指标测定结果也明显高于心功能Ⅰ~Ⅱ级患者[CRP(mg/L):13.75±3.61比3.88±1.72,CA125(U/L):65.74±7.12比14.18±2.56,c Tn(Iμg/L):0.49±0.04比0.11±0.02,P均<0.05]。结论 CRP、CA125、c Tn I 3项指标检测可以应用于CHF的临床诊断,其测定结果高低直接与患者心功能等级相关。
Objective To investigate the value of combined detection of C-reactive protein (CRP), carbohydrate antigen 125 (CA125) and cardiac troponin I (cTn I) in the diagnosis of chronic heart failure (CHF). Methods Sixty CHF patients (CHF group) treated in Dezhou Hospital of Traditional Chinese Medicine from May 2014 to May 2016 were selected and 60 healthy subjects (healthy control group) were selected for physical examination at our hospital during the same period. Fasting venous blood samples were collected on the 2nd day after fasting, and the CRP, CA125 and cTn I levels in both groups were measured. The differences of 3 indexes among CHF patients with different cardiac function were compared. Results The CRP, CA125 and cTnI indexes of CHF patients were significantly higher than those of the healthy controls [CRP (mg / L): 9.02 ± 4.06 vs 2.20 ± 1.19, CA125 (U / L): 47.95 ± 6.03 6.51 ± 0.47, cTn I (μg / L): 0.49 ± 0.04 vs 0.02 ± 0.01, P all <0.05], and the results of the above three indexes of heart function Ⅲ ~ Ⅳ CHF patients were also significantly higher than those of cardiac function Ⅰ ~ Patients with grade II [CRP (mg / L): 13.75 ± 3.61 vs 3.88 ± 1.72, CA125 (U / L): 65.74 ± 7.12 vs 14.18 ± 2.56, cTn (Iμg / L): 0.49 ± 0.04 vs. 0.11 ± 0.02, P <0.05]. Conclusion The detection of CRP, CA125 and cTnI 3 indexes can be applied to the clinical diagnosis of CHF. The level of CRP, CA125 and cTnI is directly correlated with the level of cardiac function in patients.
