目的 观察四维时空关联成像对早孕末期胎儿先天性心脏病筛查检测的诊断价值.方法 选取2016年3月~2017年3月于我院经早期初筛怀疑胎儿有先天性心脏病的孕妇94例, 并将其随机分为对照组和观察组, 每组47例 (每组患者的性别、年龄、孕周等差异无统计学差异), 两组患者均由同一位经验丰富的超声科医师为其检查.对照组采用常规二维超声法, 观察组在常规二维超声检查法的基础上联合四维时空关联成像法为患者进行检查.记录两组患者的诊断结果, 与后期随诊结果比较, 分析两组检查方法的灵敏度、特异度、准确度.结果 对照组诊断结果的灵敏度、特异度、准确度分别为58.06%、66.67%、72.34%;观察组诊断结果的灵敏度、特异度、准确度分别为90.91%、87.50%、93.62%, 结果比较差异有统计学意义, P<0.05.结论 四维时空关联成像联合常规二维超声对早孕末期胎儿先天性心脏病的检测结果更加准确, 灵敏度和特异度均高于常规二维超声法, 可以在临床中推广应用.“,”Objective:To observe the diagnostic value of four-dimensional spatio-temporal correlation imaging in the screening of fetal congenital heart disease in the early stage of early pregnancy. Methods:From March 2016 to March 2017, 94 patients with suspected high-risk pregnant women were initially screened in our hospital and randomly divided into control group and observation group, with 47 cases in each group (gender, Age, gestational age and other differences were not statistically different), both groups of patients by the same experienced ultrasound physician for its examination. The control group using conventional twodimensional ultrasound, the observation group on the basis of conventional two-dimensional ultrasound combined with fourdimensional spatio-temporal correlation imaging method for patients to check. Results:The sensitivity, specificity and accuracy of the diagnostic results of the control group were 58.06%, 66.67% and 72.34% respectively. The sensitivity, specificity and accuracy of the diagnostic results of the observation group were 90.91%, 87.50% and 93.62% respectively. The results were compared The difference was statistically significant, P<0.05. Conclusion:Fourdimensional spatio-temporal correlation imaging and conventional two-dimensional ultrasound are more accurate in detection of early fetal congenital heart disease in fetuses. Sensitivity and specificity are higher than conventional two-dimensional ultrasound and can be widely applied in clinical practice.
