目的初步探讨腮腺基底细胞腺瘤的MR特征,提高对腮腺基底细胞腺瘤的术前诊断准确率。方法回顾性分析4例经手术病理证实的腮腺基底细胞腺瘤的MR影像特点,包括数目、部位、大小、形态、边缘、信号特点、强化特征以及颈部间隙淋巴结情况。结果男性3例(75%),女性1例(25%),平均年龄47岁(37~55岁)。4例均为单发肿瘤,其中左侧1例,右侧3例。除1例累及腮腺深叶及浅叶外,其余肿瘤均位于腮腺浅叶。肿瘤平均大小约为1.98 cm×1.48 cm。1例肿瘤呈哑铃形,3例肿瘤呈类圆形。肿瘤轮廓清晰,并有包膜形成。以腮腺为标准:2例肿瘤信号均匀,T1WI呈低信号,化学位移选择饱和法(CHESS)脂肪抑制T2WI呈高信号,FSE T2WI呈低信号。1例肿瘤T1WI及T2WI均呈高低混杂信号,内有血肿信号。1例肿瘤T1WI呈低信号,脂肪抑制T2WI呈略高信号,FSE T2WI呈略低信号,周围可见小裂隙样囊变。增强扫描,所有肿瘤实质成分明显强化。1例肿瘤同侧颈血管间隙多个淋巴结肿大,但信号均匀。结论中老年患者腮腺浅叶内单侧单发轮廓清晰,边缘光整病变,实性或囊实性伴有出血,显示低信号包膜,并且实性成分明显强化,提示腮腺基底细胞腺瘤的可能。
Objective To investigate the MR features of parotid basal cell adenoma and to improve the accuracy of preoperative diagnosis of parotid basal cell adenoma. Methods The MR imaging features of 4 cases of parotid basal cell adenoma confirmed by surgery and pathology were retrospectively analyzed, including the number, location, size, shape, margins, signal characteristics, enhancement features and lymph nodes of the neck. Results Three males (75%) and one females (25%) had an average age of 47 years (37-55 years). 4 cases were single tumor, including 1 case on the left and 3 cases on the right. Except for 1 case involving parotid deep leaves and shallow leaves, the remaining tumors are located in the parotid shin. The average tumor size is about 1.98 cm × 1.48 cm. One patient showed a dumbbell shape and three had a round shape. Clear tumor outline, and envelope formation. Taken parotid gland as a standard, two tumor signals were homogeneous, T1WI showed low signal, CHESS fat suppressed T2WI showed high signal and FSE T2WI showed low signal. One case of tumor T1WI and T2WI were mixed signal level, there is a hematoma signal. One tumor showed a low signal on T1WI, a slightly high signal on fat-suppressed T2WI, and a slightly lower signal on FSE T2WI. Enhanced scan, all the substantial enhancement of tumor components. One case had multiple lymphadenopathy of ipsilateral neck vascular space, but the signal was even. Conclusions In middle-aged and elderly patients, the unilateral single hair follicles in the parotid gland lobes have clear contours, marginal eyelid lesions, solid or cystic solid with hemorrhage, low signal envelopes, and significant enhancement of the solid components, suggesting that parotid basal cell adenoma may.
