目的探讨吸入一氧化氮(NO)治疗不同类型的肺动脉高压的适应证及疗效。方法采用BG-951型一氧化氮治疗仪治疗先天性心脏病(CHD)伴肺动脉高压(PH)惠儿81例、高原性肺水肿22例、新生地持续肺动脉高压(PPHN)10例、肺心病20例、哮喘10例,共计5种类型143例患者。结果对CHD伴PH患儿可显著降低PAP(P<0.01),对高原肺水肿患者肺部湿性罗音和X线胸片阴影消失时间比常规治疗显著缩短(P<0.01),对PPHN患儿氧合指数显著改善,肺动脉压力降低(P<0.05),对肺心病血氧分压及血氧饱和度明显改变(P<0.05),对10例哮喘患者肺通气功能改变率为20%有4例,改变率为10%有3例。结论BG-951型一氧化氮治疗仪性能稳定可靠,治疗多种肺动脉高压安全、有效。
Objective To investigate the indications and therapeutic effects of inhaled nitric oxide (NO) in the treatment of different types of pulmonary hypertension. Methods 81 cases of congenital heart disease (CHD) with pulmonary hypertension (PH), 22 cases of high altitude pulmonary edema, 10 cases of persistent pulmonary hypertension (PPHN) in Xinsheng, and pulmonary heart disease were treated with the BG-951 nitric oxide therapy apparatus. 20 cases and 10 cases of asthma, a total of 5 types of 143 patients. Results PAP was significantly reduced in children with CHD and PH (P<0.01), and the time of disappearance of lung wet rales and X-ray chest shadow was significantly shorter in patients with high altitude pulmonary edema than in conventional treatment (P<0.01). Oxygenation index of children with PPHN was significantly improved, pulmonary artery pressure was decreased (P<0.05), oxygen partial pressure and oxygen saturation of pulmonary heart disease were significantly changed (P<0.05), and pulmonary ventilation was performed in 10 asthmatic patients. There were 4 cases of functional change rate of 20% and 3 cases of change rate of 10%. Conclusion The performance of the BG-951 nitric oxide therapy apparatus is stable and reliable, and it is safe and effective in treating multiple pulmonary hypertension.
