目的 :探讨西洛他唑预防犬下腔静脉狭窄内置支架术后再狭窄的影响。方法 :制作成下腔静脉狭窄模型的健康成年杂种犬 18条 ,随机分为 A(实验组 )、B(对照组 )两组 ,每组 8条 ,采用 Seldinger技术经股静脉径路扩张狭窄的下腔静脉并置入国产 2 0 35自膨式 Z型金属支架。A、B两组手术前 10 d至手术后 4周分别服用西洛他唑及阿司匹林肠溶片。术后第 4周时造影后获取标本作组织学检查。血管造影观察静脉通畅性、下腔静脉压力差、最小血管直径情况及支架是否移位 ;HE染色及组织化学检查了解内膜增生、胶原沉积及平滑肌α- Actin表达情况。扫描电镜评估内皮层变化 ;透射电镜观察平滑肌细胞增生情况。结果 :全组支架均成功置入到狭窄的下腔静脉处 ,并且保持通畅 ,腔静脉直径有了一定的增宽。A组的下腔静脉压力差降低及最小血管直径较 B组好 ( P <0 .0 5 )。光镜下见 A组内膜增生、胶原纤维染色、平滑肌 α- Actin染色均较 B组轻 ;电镜显示 A组内皮层尚完整 ,平滑肌细胞呈长菱形 ,核异染色质多 ,形态欠规则 ,胞质有少量细胞器 ,充满细丝 ,有密体和密斑 ,质膜外有基板。B组则见平滑肌细胞粗面内质网扩张。结论 :西洛他唑作为一种 c AMP-磷酸二酯酶的抑制剂 ,可通过减轻血管内膜增生及平滑肌细胞增殖来减少再狭窄的?
Objective: To investigate the effect of cilostazol on prevention of restenosis after stent implantation in dogs with inferior vena cava stenosis. Methods: Eighteen healthy adult dogs were made into inferior vena cava stenosis model. They were randomly divided into A (experimental group) and B (control group), with 8 in each group. Seldinger technique was used Vena Cava and placed in China 2 0 35 self-expanding Z-shaped metal stent. Groups A and B were given cilostazol and aspirin enteric-coated tablets 10 days before surgery and 4 weeks after surgery respectively. After 4 weeks after surgery, the specimens were obtained for histological examination. The venous patency, inferior vena cava pressure difference, minimum vessel diameter and stent displacement were observed by angiography. HE staining and histochemical examination were used to detect the expression of α-actin in intimal hyperplasia, collagen deposition and smooth muscle. Scanning electron microscopy assessment of endothelial changes; transmission electron microscopy of smooth muscle cell proliferation. Results: All the stents were successfully implanted into the narrow IVC and remained unobstructed. The diameter of the vena cava had been widened. A group of inferior vena cava pressure drop and the minimum vascular diameter better than the B group (P <0. 05). A group under light microscope, see the group of intimal hyperplasia, collagen staining, smooth muscle a-Actin staining were lighter than the B group; electron microscopy showed A group of endothelial cells is still intact, smooth muscle cells were rhombohedrosis, nuclear heterochromatin and morphology irregular, Cytoplasm with a small amount of organelles, full of filaments, dense and plaque, the plasma membrane with a substrate. Group B see the smooth muscle cells rough endoplasmic reticulum dilatation. CONCLUSIONS: Cilostazol, as an inhibitor of c AMP-phosphodiesterase, reduces restenosis by reducing intimal hyperplasia and smooth muscle cell proliferation.
