目的对纯音测听(pure tone audiometry,PTA)、听性脑干反应(auditory brain stem response,ABR)正常的唇腭裂患者行听觉失匹配负波(auditory mismatch negativity,AMMN)检查,并与健康正常人对照,判断唇腭裂患者听觉皮层的功能状态,为探讨其言语功能障碍机制提供理论依据。方法唇裂伴有腭裂(Cleft Lip and Palate,CLP)39例,单纯唇裂(Cleft Lip,CL)28例,单纯腭裂(Cleft Palate,CP)17例,健康志愿者(正常组)39例(纯音测听、听性脑干反应均正常)分别行AMMN检查,比较各组间潜伏期及波幅的差异。结果 CP组AMMN潜伏期较正常组、CL组显著延长(P<0.05),CP组潜伏期较CLP组无明显差异(P>0.05);CLP组潜伏期较CL组、正常组显著延长(P<0.05);CL组潜伏期与正常组无明显差异(P>0.05)。各组间AMMN的波幅无明显差异(P>0.05)。结论 CP及CLP患者听觉皮层的功能可能存在异常,可能为导致其言语功能障碍的一个原因,但仍然需要结合其言语识别能力、言语功能评定、磁共振(magnetic resonance,MR)、功能性磁共振(functional magnetic resonance imaging,fMRI)加以进一步研究。
Objective To investigate auditory mismatch negativity (AMMN) in pure tone audiometry (PTA) and auditory brain stem response (ABR) patients with normal cleft lip and palate, and to compare with healthy normal People control, to determine the functional status of auditory cortex in patients with cleft lip and palate, to provide a theoretical basis for exploring the mechanism of speech dysfunction. Methods Cleft Lip and Palate (CLP) were performed in 39 cases, Cleft Lip (CL) in 28 cases, Cleft Palate (CP) in 17 cases and healthy volunteers (normal group) in 39 cases Listening, listening, brainstem response were normal) AMMN were examined, compared between the differences in latency and amplitude. Results The latency of AMMN in CP group was significantly longer than that in normal group and CL group (P <0.05), and latency of CP group was no significant difference (P> 0.05). The latency of CLP group was longer than that of CL group and normal group (P <0.05) There was no significant difference between CL group and normal group (P> 0.05). There was no significant difference in amplitude of AMMN between groups (P> 0.05). Conclusions The function of auditory cortex may be abnormal in CP and CLP patients, which may be one of the causes of speech dysfunction. However, it is still necessary to combine the speech recognition ability, speech function evaluation, magnetic resonance (MR), functional magnetic resonance (functional magnetic resonance imaging, fMRI) for further study.
