目的 :研究FAB分型及免疫表型在过氧化物酶染色 (POX)阴性急性白血病 (AL)诊断中的诊断价值。方法 :先取POX阴性AL住院患者 ,选择符合条件的病例 ,骨髓涂片经瑞氏染色后分类计数 ,并经POX、PAS、CE、AE +NaF、NAP积分等细胞化学染色检查 ;免疫表型采用流式细胞仪测定 ;根据EGIL之积分标准诊断杂合型急性白血病。结果 :3 8例POX阴性AL患者 ,FAB分型诊断为ALL 3 3例 ,AML 5例 ;免疫表型诊断为ALL 2 8例 ,AML 6例 ,HAL 4例。结论 :POX不能作为区分AML和ALL的惟一凭据 ,对POX阴性的AL ,有必要予CE、AE +NaF、NAP积分等其他化学染色结合免疫表型检查 ,才能提高诊断的准确性 ,以免误诊
Objective: To investigate the diagnostic value of FAB typing and immunophenotype in the diagnosis of peroxidase-stain (POX) -dependent acute leukemia (AL). Methods: The patients with POX-negative AL were selected in our hospital and selected eligible patients. The bone marrow smears were sorted by Wright’s stain and counted by cytochemical staining such as POX, PAS, CE, AE + NaF and NAP. Immunophenotype Flow cytometry; diagnosis of heterozygous acute leukemia based on the EGIL integral criteria. Results: A total of 38 ALL cases were diagnosed by FAB typing and 5 cases were AML cases in 38 POX-negative AL patients. The immunophenotypes were ALL 28 cases, AML 6 cases and HAL 4 cases. Conclusion: POX can not be used as the only evidence to differentiate AML and ALL. For POX-negative AL, it is necessary to apply other chemical staining such as CE, AE + NaF, NAP integral and other immunophenotype examination in order to improve the diagnostic accuracy so as to avoid misdiagnosis
