目的:通过24h动态心电图监测心房颤动患者的心率变化,了解安装起搏器的时机。方法:随机抽取2008年1月—2011年1月进行动态心电图监测获得心房颤动的患者350例,将其分为持续性心房颤动180例,阵发性心房颤动170例。结果:持续性心房颤动组出现R-R间期>1.5s共321次,白天发生70次,夜间发生251次,恢复后150例出现传导阻滞,30例发生P-R间期延长,12例于夜间12时至次日凌晨6时发生传导阻滞;阵发性心房颤动组出现R-R间期>1.5s的次数为290次,白天发生35次,夜间发生135次,恢复后有150例为出现传导阻滞,20例患者出现P-R间期延长,10例患者于夜间11时至次日凌晨5时发生传导阻滞。结论:使用24h动态心电图对心房颤动患者进行监测,能够有效监测其心率变化情况,提高传导阻滞诊断的准确率。
OBJECTIVE: To monitor the change of heart rate in patients with atrial fibrillation by 24h Holter to understand the timing of pacemaker installation. Methods: A total of 350 patients with atrial fibrillation who underwent ambulatory ECG monitoring from January 2008 to January 2011 were randomly divided into 180 cases of persistent atrial fibrillation and 170 cases of paroxysmal atrial fibrillation. Results: In the patients with persistent atrial fibrillation, there were 321 episodes with RR interval> 1.5s, 70 episodes during daytime, 251 nights during remission, and 150 cases after block. The PR interval prolonged in 30 patients and 12 in nighttime When the next day 6:00 occurred block; paroxysmal atrial fibrillation group RR interval> 1.5s the number of 290 times, occurred 35 times during the day and night occurred 135 times, 150 cases after recovery for the emergence of resistance Hysteresis, 20 cases of patients with prolonged PR interval, 10 patients at 11 pm to 5 am the next day occurred block. Conclusion: Monitoring the atrial fibrillation patient with 24h dynamic electrocardiogram can effectively monitor the change of heart rate and improve the diagnostic accuracy of conduction block.
