目的分析瓦斯爆炸后吸入性损伤影像表现的动态变化特点,探讨影像随访的意义。方法回顾性分析17例瓦斯爆炸吸入性损伤不同治疗阶段的影像表现,随访分别于发病后2 h及2、4、8、13、18、30d进行。结果吸入性损伤影像表现与临床发展转归密切相关,早期表现为肺水肿、肺渗出性改变或肺不张,即表现为肺纹理增多或淡薄、斑片状的磨玻璃样改变,斑片状、团絮样模糊的渗出性病灶,散在性、点片状渗出灶,或部分肺实变;继而并发肺部感染,并随治疗转归而逐渐吸收,17例全部治愈出院。结论吸入性损伤的影像动态观察能为临床提供可靠的治疗依据,可早期诊断吸入性损伤。
Objective To analyze the dynamic change characteristics of the image of inhalation injury after gas explosion and discuss the significance of image follow-up. Methods We retrospectively analyzed the imaging manifestations of 17 cases of inhalation injury of gas explosion in different treatment stages. The follow-up was performed at 2 h and 2, 4, 8, 13, 18 and 30 days after onset. Results The imaging findings of inhalation injury were closely related to the clinical development and prognosis. The early manifestation of pulmonary edema, pulmonary exudative changes or atelectasis showed the increase or weakness of the lungs, the patchy ground glass-like changes, Like lumps like blurry exudative lesions, scattered, punctate exudative foci, or part of the lung consolidation; and then complicated by pulmonary infection, and gradually transferred with the treatment outcome, 17 cases were cured. Conclusions The dynamic observation of inhalation injury can provide a reliable basis for clinical treatment and early diagnosis of inhalation injury.
