[目的]应用实时荧光定量RT-PCR检测非小细胞肺癌(NSCLC)患者外周血CK19mRNA和LUNXmRNA,探讨其诊断NSCLC外周血微转移的临床意义。[方法]选取初治NSCLC患者40例及健康志愿者50例,用实时荧光定量RT-PCR检测外周血CK19mRNA和LUNXmRNA相对定量分析结果。[结果](1)肺癌组与健康对照组的CK19mRNA阳性表达率及相对表达量均有显著差异;肺癌组阳性表达率与原发肿瘤部位、原发肿瘤大小、临床分期、分化程度及病理类型无相关性,其相对表达量鳞癌患者高于腺癌患者(P<0.05)。(2)肺癌组与健康对照组的LUNXmRNA阳性表达率有显著差异,其表达与临床病理特征无相关性。[结论]CK19mRNA和LUNXmRNA作为实时荧光定量RT-PCR法检测NSCLC外周血微转移的分子标志物,具有一定临床应用价值。
[Objective] To detect the expression of CK19mRNA and LUNXmRNA in peripheral blood of patients with non-small cell lung cancer (NSCLC) by real-time fluorescent quantitative RT-PCR and to explore its clinical significance in the diagnosis of NSCLC peripheral blood micrometastasis. [Methods] Forty patients with primary NSCLC and 50 healthy volunteers were enrolled. Relative quantitative analysis of CK19 mRNA and LUNX mRNA in peripheral blood was performed by real-time fluorescent quantitative RT-PCR. [Results] (1) The positive expression rate and relative expression of CK19 mRNA in lung cancer group and healthy control group were significantly different. The positive expression rate of lung cancer group was significantly correlated with the size of primary tumor, tumor size, clinical stage, differentiation and pathological type No correlation was found between the relative expression of squamous cell carcinoma and adenocarcinoma patients (P <0.05). (2) The positive rates of LUNXmRNA in lung cancer group and healthy control group were significantly different, the expression of which had no correlation with clinicopathological features. [Conclusion] As a molecular marker of micrometastasis in peripheral blood of NSCLC, CK19 mRNA and LUNX mRNA have certain clinical value as real-time fluorescent quantitative RT-PCR.
