目的 评价溴芬酸钠滴眼液治疗白内障术后眼部炎症的有效性和安全性.方法 计算机检索Pub Med、Medline、OVID、EMBase、Cochrane图书馆、Springer Link、中国知网、维普、万方等数据库中关于溴芬酸钠滴眼液治疗白内障术后眼部炎症有效性和安全性的随机对照试验 (RCTs), 检索时限均为建库至2017年10月.采用Rev Man 5.3对纳入的文献进行Meta分析.结果 共纳入7篇RCTs, 3 514例单眼接受白内障手术患者.分析结果显示溴芬酸钠滴眼液组在白内障手术15 d后实现眼部炎症完全清除的受试者比率和在白内障手术后第1日眼部疼痛得到缓解的受试者比率高于安慰剂组[分别为RR=1.94, 95%CI (1.72, 2.19), P<0.000 01;RR=1.35, 95%CI (1.21, 1.50), P<0.000 01];溴芬酸钠滴眼液组因缺乏疗效或不良反应而终止临床试验的受试者数量显著低于安慰剂组[分别为RR=0.14, 95%CI (0.11, 0.18), P<0.000 01;RR=0.32, 95%CI (0.24, 0.42), P<0.000 01].溴芬酸钠滴眼液组的总不良反应发生率、眼痛和畏光的发生率低于对安慰剂组 (P<0.000 01), 其他不良反应 (眼压升高、头疼、异物感、结膜充血、严重不良反应) 的发生率两组间均无显著差异 (P>0.05) .结论 溴芬酸钠滴眼液用于白内障术后能有效改善眼部炎症、眼部疼痛, 其不良反应少且耐受.“,”AIM To evaluate the efficacy and safety of bromfenac sodium eye drops on ocular inflammation following cataract surgery.METHODS Pub Med, Medline, OVID, EMBase, Cochrane Library, CNKI, Wanfang database were searched to collect the literatures on randomized controlled trials (RCTs) about bromfenac sodium eye drops on ocular inflammation following cataract surgery.Meta-analysis was conducted by Rev Man 5.3 software.RESULTS A total of 7 RCTs were included (n = 3 514).Compared with the placebo, a greater proportion in bromfenac sodium eye drops achieved complete clearance of ocular inflammationat 15 days after cataract surgery (RR = 1.94, 95% CI (1.72, 2.19), P < 0.000 01).The proportion of pain-free in the bromfenac sodium eye drops group was significantly higher than the placebo group at 1 day after treatment[RR = 1.35, 95% CI (1.21, 1.50), P < 0.000 01].The number of subjects who were discontinued due to the lack of efficacy and adverse effects was significantly lower in the bromfenac sodium eye drops group than the placebo group [RR = 0.14, 95% CI (0.11, 0.18), P < 0.000 01;RR = 0.32, 95% CI (0.24, 0.42), P <0.000 01].The incidence of total adverse reactions, eye pain and photophobia in the bromfenac sodium eye drops group was lower than the placebo group (P < 0.000 01).There were no significant differences between the two groups in the incidence of other adverse reactions (ocular hypertension, headache, foreign body sensation, conjunctival hyperemia, serious adverse reactions) (P> 0.05).CONCLUSION Bromfenac sodium eye drops could improve the postoperative inflammation and ocular pain relief of cataract, with lower adverse drug reactions and generally well tolerated.
