目的探讨并观察白内障超声乳化联合人工晶体植入手术对糖尿病视网膜病变的影响。方法对40例2型糖尿病白内障患者,1眼行白内障超声乳化联合人工晶体植入术为手术组,对侧非手术眼为对照组。分别于术前1天、术后第1天、术后1、2周、术后1、2、3个月行双眼底彩色照相,并于术前、术后1个月、术后3个月行眼底荧光血管造影检查确定糖尿病视网膜病变的分期及进展情况。结果术后1个月糖尿病视网膜病变有进展的手术组10眼(25%),非手术组3眼(7.5%);术后3个月糖尿病视网膜病变有进展的手术组36眼(90%),非手术组7眼(17.5%)。结论白内障超声乳化手术会加快糖尿病视网膜病变的发展进程,应密切观察白内障术后视网膜的变化,及时行眼底激光治疗,减缓糖尿病视网膜病变的进展。
Objective To investigate and observe the effect of phacoemulsification combined with intraocular lens implantation on diabetic retinopathy. Methods Forty cases of type 2 diabetic cataract patients underwent cataract phacoemulsification and intraocular lens implantation. The contralateral non-surgical eyes served as the control group. Bilateral fundus color pictures were taken at 1 day before surgery, 1 day after surgery, 1 week after surgery, 1 week after surgery, 1 and 2 months after surgery respectively. Preoperative and postoperative 1 month, postoperative 3 Fundus fluorescein angiography examination confirmed the stage and progress of diabetic retinopathy. Results One month after operation, 10 eyes (25%) had progressive diabetic retinopathy and 3 eyes (7.5%) had non-operation diabetic retinopathy. Thirty-six eyes (90%) developed diabetic retinopathy at 3 months after operation. , Non-surgical group of 7 eyes (17.5%). Conclusions Cataract phacoemulsification can speed up the development of diabetic retinopathy. Retinal changes after cataract surgery should be closely monitored. Fundus laser treatment should be performed in time to slow down the progression of diabetic retinopathy.
