目的分析研讨超声“四切面”法筛查胎儿心脏大动脉根部平行走向先心病的临床作用。方法本次研讨中均随机从2016年1月至2017年1月期间我院接受的超声产检孕妇450例,采用超声“四切面”法进行先天性心脏病筛查,其结果与超声心动图和单纯心腔的心切法进行对比。结果 450例经超声“四切面”法筛查出大动脉根部走向特征的先心病胎儿10例;10例胎儿经超声四切面法与超声心动图检查对比后,1例未见明显异常,其检出9例(90%);10例胎儿经四切面法与单纯的四腔心切面检查结果对比,8例为做室流出道切面检查发现大动脉根部平行而检出的,敏感性为80%(8/10),其中6例在四腔心切面中未发现明显异常,3例为显示室缺。10例单纯的四腔心切面中发现异常只有6例,其敏感性60%(6/10)。结论超声“四切面”法筛查胎儿心脏大动脉根部平行走向先心病,有着很高的敏感性与重要提示的作用,再与胎儿超声心动图相结合,可以进一步提高该类CHD的诊断,有临床应用价值。
Objective To analyze and discuss the clinical effect of ultrasonography and “four-slice” method in screening the parallel movement of fetal aortic root to congenital heart disease. Methods In this study, 450 pregnant women undergoing ultrasound examination in our hospital were randomly selected from January 2016 to January 2017. The screening of congenital heart disease was performed by the method of ultrasound and the “four-slice” method. The results were compared with those of echocardiography Figure and simple heart cavity cardiometry comparison. Results Among the 450 cases, 10 cases of congenital heart disease were screened out by ultrasound and the “four-section method” method. One case had no obvious abnormality after 10 cases of fetus were examined by echocardiography and echocardiography 9 cases (90%) were detected in 10 cases. The results of four-section method in 10 fetuses were compared with those in simple four-chamber heart-grafts. Eight cases were detected by parallel examination of aorta outflow tract with a sensitivity of 80% (8/10), of which 6 cases showed no obvious abnormalities in the four-chamber coronal section and 3 cases showed ventricular septal defect. In only 10 cases of simple four-chamber coronal section found only 6 cases of abnormality, the sensitivity of 60% (6/10). Conclusion Ultrasound “four-slice” method to screen fetal heart aortic root parallel to congenital heart disease, has a high sensitivity and important tips, and then combined with fetal echocardiography can further improve the diagnosis of such CHD, Clinical application of value.
