目的:研发抗菌药使用剂量和频次智能化监控系统(以下简称“系统”)以减少相关不良事件和提高抗感染疗效。方法:基于医院信息系统,实现抗菌药每日使用剂量和频次与患者年龄、肾功能状况相链接。结果:随机抽取1d数据,在516张住院患者的抗菌药处方中,“系统”共出现45条警示。对肾功能正常的成人,时间依赖性抗菌药以每日使用频次不足居多;而对肾功能不全者或老年人,则均为每日使用剂量或频次过多,发现因此而引发的药物不良事件2例。结论:这种链接方式切实可行,当抗菌药物处方中出现不符合说明书规定的使用剂量和(或)频次时,能够被该系统识别,而其临床作用有待实践检验。
OBJECTIVE: To develop an intelligent monitoring system for dosage and frequency of antimicrobial agents (hereinafter referred to as “system”) to reduce related adverse events and improve anti-infective efficacy. Methods: Based on the hospital information system, daily doses and frequency of antibacterials were linked with the patient’s age and renal function. Results: 1d data were randomly selected. Of the 516 antibacterial prescriptions for hospitalized patients, there were 45 warnings in “System”. For adults with normal renal function, time-dependent antibacterials are mostly used less frequently per day; whereas in patients with renal insufficiency or in the elderly, daily doses or frequency of overdose are used, and the resulting adverse drug events are found 2 cases. CONCLUSIONS: This link approach is viable and can be identified by the system when an antibacterial drug formulation does not meet the prescribed dosage and / or frequency prescribed in the instructions, and its clinical role remains to be tested.
