目的 研究子疒间 前期患者的左心室结构和收缩功能变化及其与血浆脑利钠肽 (BNP)水平的相关性。方法 1999年 3月至 2 0 0 3年 3月间 ,研究上海第二医科大学仁济医院子疒间 前期患者 4 6例 ,并随机选择同期正常晚期妊娠孕妇 32例作为对照。采用彩色多普勒超声测定左心室结构指标和收缩功能指标 ,用放射免疫方法测定血浆脑利钠肽浓度。结果 子疒间 前期患者心脏室间隔厚度、左室舒张末期内径和容积、收缩末期内径和容积、左室后壁厚度、左心室重量和重量指数均显著大于正常妊娠组 (P <0 0 1)。患者的BNP物质的质量浓度 (4 4 34±37 6 9)pmol L显著高于正常晚期妊娠孕妇 (12 6 9± 8 84 )pmol L(P <0 0 1) )。子疒间 前期患者和正常妊娠孕妇的左心室收缩功能敏感指标左心室射血分数 (EF)与血浆脑利钠肽物质的量浓度均存在高度相关性。结论 子疒间 前期患者的左心室室壁明显肥厚、内径和容积扩大 ,收缩功能显著受损。血浆脑利钠肽浓度是反应子疒间 前期患者左心室收缩功能状态的敏感指标。
Objective To study the changes of left ventricular structure and systolic function in patients with preeclampsia and its relationship with plasma brain natriuretic peptide (BNP) levels. Methods Between March 1999 and March 2003, 46 cases of pre-eminence patients in Renji Hospital of Shanghai Second Medical University were studied, and 32 pregnant women with normal late pregnancy during the same period were randomly selected as the control. Left ventricular structural indexes and systolic function indexes were measured by color Doppler sonography, and plasma concentrations of brain natriuretic peptide were measured by radioimmunoassay. Results The thickness of interventricular septum, left ventricular end-diastolic diameter and volume, end-systolic diameter and volume, left ventricular posterior wall thickness, left ventricular weight and weight index in preeclampsia were significantly higher than those in normal pregnancy group (P <0.01) . The BNP substance mass concentration (4 4 34 ± 37 6 9) pmol L in patients was significantly higher than that in normal late pregnant women (12 6 9 ± 8 84) pmol L (P 0 01). There was a significant correlation between left ventricular ejection fraction (EF) and plasma concentration of brain natriuretic peptide in patients with preeclampsia and normal pregnant women. Conclusion The preeclampsia patients with left ventricular wall was significantly hypertrophy, diameter and volume expansion, constrictive function was significantly impaired. Plasma brain natriuretic peptide concentration is a sensitive indicator of left ventricular systolic function in preeclampsia.
