目的 探索一种能精确诊断踝关节前后脱位的X线量化分析指标。 方法 根据生物力学与几何学原理 ,设计一种能在踝关节侧位X线片上进行量化分析的“距骨中心中轴距”(L值 )测量方法 ;筛选 2 0 0名健康成人进行L正常值范围测定 ,并对 1 865例 1 92 0侧踝部损伤病例做回顾分析。 结果 2 0 0名健康成人的L值为 0 .0~ 2 .5mm[(0 .84± 0 .59)mm] ,其 95 %位数 (P95)为 1 .99mm ;以L值 >2mm为诊断标准 ,踝关节前后脱位组的阳性诊断率即诊断灵敏度为 1 0 0 .0 %。结论 L值可望作为踝关节前后脱位的一个精确诊断标准 ,值得做进一步的临床研究
Objective To explore an X-ray quantitative analysis index that can accurately diagnose anterior and posterior ankle dislocations. Methods According to the principles of biomechanics and geometry, we designed a method for measuring the “central axis distance of the talus” (L value) which can be quantitatively analyzed on the lateral ankle X-ray film. Two hundred and fifty healthy adults were screened for L normal Range of determination, and 1 865 cases of 1 92 0 lateral ankle injury cases do review analysis. Results The L value of 200 healthy adults was 0 .0-2.5 mm [(0. 84 ± 0. 59) mm], and the 95% figure (P95) was 1.99 mm. The L value> 2 mm was Diagnostic criteria, positive ankle anterior dislocation diagnosis rate that diagnostic sensitivity was 100.0%. Conclusion L value is expected to be an accurate diagnostic criteria for ankle anterior and posterior dislocation, it is worth further clinical study
