目的探讨YKL-40、Survivin在前列腺癌中的表达及与临床病理学参数的关系,评价二者对判断患者预后的价值。方法采用免a疫组织化学EnVision法检测56例前列腺癌中YKL-40、Survivin的表达情况。用Kaplan-Meier法分析生存曲线,多变量Cox比例风险回归模型筛选影响患者生存的独立预后因素。结果56例前列腺癌中,YKL-40阳性表达41例(73%),Survivin阳性表达45例(80%),二者表达呈正相关,共同阳性表达34例(61%);与Gleason评分及骨转移有关(P<0.05)。YKL-40阳性、Survivin阳性、共同阳性表达及Gleason积分高的患者术后生存时间明显缩短,Cox多因素分析表明YKL-40与Gleason评分是影响患者生存的独立预后因素。结论YKL-40、Survivin在前列腺癌中普遍高表达,与Gleason评分、临床分期、骨转移及患者术后生存有关,有助于判断前列腺癌的恶性程度、监测患者预后。
Objective To investigate the relationship between the expression of YKL-40 and Survivin in prostate cancer and the clinicopathological parameters, and to evaluate their value in judging the prognosis of patients. Methods The immunohistochemical EnVision method was used to detect the expression of YKL-40 and Survivin in 56 cases of prostate cancer. Survival curves were analyzed by Kaplan-Meier method, and multivariate Cox proportional hazards regression model was used to screen for independent prognostic factors affecting patient survival. Results The positive expression of YKL-40 in 41 cases (73%) and the positive expression of Survivin in 56 cases of prostate cancer were 45 cases (80%). The positive expression of YKL-40 was positive in 34 cases Metastasis related (P <0.05). The survival time of patients with YKL-40 positive, Survivin positive, co-positive expression and high Gleason score was significantly shortened. Cox multivariate analysis showed that YKL-40 and Gleason score were independent prognostic factors in patients’ survival. Conclusion The expression of YKL-40 and Survivin in prostate cancer is generally high, which is correlated with Gleason score, clinical stage, bone metastasis and postoperative survival of patients. It is helpful to judge the malignant degree of prostate cancer and monitor the prognosis of patients.
