目的 :探讨菌阳肺结核患者化疗前后其血清结核抗体与痰菌转归及预后的关系。方法 :化疗前及化疗期 ,每月检测血清特异性结核抗体 PPD- Ig G(EL ISA法 ) ,同时做细菌学检查 (涂片 +培养 )。停药后继续追踪两年。结果 :74例中 ,结核抗体 (Anti+TB)呈持续强阳性反应者 2例 (复治 ) ,化疗无效。余 72例 ,结核抗体与结核菌之间显示出五组关系 ,1抗体与痰菌同时转阴 ,占 5 .5 6% (4/72 ) ;2痰菌转阴 ,抗体仍阳性 ,占 44 .44 % (3 2 /72 ) ;3化疗后抗体才转阳 ,占 16.67% (12 /72 ) ;4抗体持续阴性 ,占 2 6.3 9% (19/72 ) ;5抗体先于痰菌转阴 ,占 5 .5 6% (4/72 )。痰菌转阴时间 :第一、五组最长 ;初治者第四组时间最短 ,平均 1.46个月 ;复治比初治转阴时间长 ;复发病例仅见于二、三组。结论 :1抗体早转阴者 ,痰结核菌转阴时间晚 ;2抗体比痰菌晚阴者 ,有结核复发可能 ;3抗体阴性 :初治者 ,抗结核疗效好 ,复治者则反之 ;4抗体持续强阳性者 ,痰菌难以转阴
Objective: To investigate the relationship between serum TB antibody and sputum bacteremia and prognosis in patients with positive pulmonary tuberculosis before and after chemotherapy. Methods: Before and during chemotherapy, serum specific tuberculosis antibody PPD-Ig G (EL ISA method) was tested every month, and bacteriological examination (smear + culture) was done at the same time. After discontinuation continue to track for two years. Results: In 74 cases, anti-TB antibody showed continuous strong positive reaction in 2 cases (retreatment), chemotherapy was ineffective. Mycobacterium tuberculosis and tuberculosis showed a relationship between the five groups, 1 antibody and sputum simultaneously negative, accounting for 5.66% (4/72); 2 sputum negative, the antibody is still positive, accounting for 44 .4 antibody (3 2/72); 3 antibody positive after chemotherapy, accounting for 16.67% (12/72); 4 antibody negative, accounting for 2 6.39% (19/72); 5 antibody before the sputum transfer Overcast, accounting for 5.6% (4/72). The sputum negative conversion time: the first and fifth groups were the longest; the fourth group of the first treatment was the shortest, with an average of 1.46 months; the retreatment was longer than the first treatment and the recurrence was only seen in the second and third groups. Conclusion: 1 antibody early negative people, sputum TB bacilli late; 2 antibodies than sputum bacteria late Yin, tuberculosis recurrence possible; 3 antibody negative: naive, anti-TB efficacy is good, 4 antibodies continued strong positive, sputum difficult to negative
