患儿女性,11岁,平素体健。行破伤风类毒素接种(0.5ml)后20h 出现双下肢麻木,以后渐出现视力减退和肢体无力。发病3天后入院。检查:双侧视神经炎和下肢痉挛性轻瘫。左眼仅有光感,右眼光感消失。双侧视盘肿胀。发病第5～6日,下肢瘫痪加重(左0/5,右1/5级)。第12日T_4水平以下感觉消失,包括振动觉和位置觉。脑脊液(CSF)蛋白48mg/dl,无细胞及血液;病毒学检查阴性,未出现寡克隆带。头颅CT 和脊髓磁共振影像(MRI)正常。
Children with children, 11 years old, usually physical health. Line tetanus toxoid inoculation (0.5ml) 20h after emergence of lower extremity numbness, and later gradually decreased visual acuity and limb weakness. 3 days after onset of admission. Check: bilateral optic neuritis and lower extremity spastic paresis. The left eye only light sense, right eye light sense disappears. Bilateral swelling of the optic disc. The incidence of 5 to 6 days, lower limb paralysis increased (left 0/5, right 1/5 level). On the 12th day, the level of T_4 below the feeling disappears, including vibration and position perception. Cerebrospinal fluid (CSF) protein 48mg / dl, no cells and blood; Virology negative, no oligoclonal bands appear. Head CT and spinal magnetic resonance imaging (MRI) normal.
