目的探讨胰腺癌组织中尿激酶型纤溶酶原激活物(urokinase-type plasminogen activator,uPA)和细胞增殖核抗原Ki-67蛋白的表达情况。方法胰腺癌患者63例,取手术切除胰腺癌组织63份为胰腺癌组,癌旁非肿瘤胰腺组织11份为对照组,采用免疫组织化学法检测2组uPA和Ki-67蛋白的阳性表达情况,分析uPA、Ki-67蛋白阳性表达与临床病理特征的关系。结果胰腺癌组uPA和Ki-67蛋白表达阳性率分别为65.1%和74.6%,对照组表达均为阴性;胰腺癌组uPA蛋白阳性表达率在肿瘤分化程度、TNM分期、有无淋巴结转移上差异有统计学意义(P<0.05),Ki-67蛋白阳性表达率在肿瘤分化程度、TNM分期、有无淋巴结转移、有无远处转移上差异有统计学意义(P<0.05),uPA和Ki-67蛋白阳性表达在年龄、性别、肿瘤直径、肿瘤部位上差异无统计学意义(P>0.05);Spearman相关分析结果显示,uPA蛋白阳性表达与肿瘤分化程度呈负相关(r=-0.333,P=0.008),与TNM分期及淋巴结转移呈正相关(r=0.430,P<0.001;r=0.400,P=0.001);Ki-67蛋白阳性表达与肿瘤分化程度呈负相关(r=-0.333,P=0.008),与TNM分期、远处转移及淋巴结转移呈正相关(r=0.494,P<0.001;r=0.340,P=0.006;r=0.400,P=0.001);Spearman关联性分析结果显示,胰腺癌组织uPA和Ki-67蛋白阳性表达呈正关联(r=0.261,P=0.032)。结论 uPA和Ki-67蛋白阳性表达可能与胰腺癌的侵袭及转移有关,可作为胰腺癌诊断、分期及预后评估的参考指标之一。
Objective To investigate the expression of urokinase-type plasminogen activator (uPA) and Ki-67 in pancreatic carcinoma. Methods Sixty-three patients with pancreatic cancer were divided into three groups: pancreatic cancer group (63 cases) and non-tumor non-tumor pancreatic tissue group (11 cases) as control group. Immunohistochemistry was used to detect the positive expression of uPA and Ki-67 , The relationship between the expression of uPA and Ki-67 protein and clinicopathological features was analyzed. Results The positive rates of uPA and Ki-67 protein in pancreatic cancer group were 65.1% and 74.6%, respectively. The positive expression rate of uPA protein in pancreatic cancer group was significantly lower than that in the differentiation degree, TNM stage and lymph node metastasis (P <0.05). The positive expression rate of Ki-67 protein in tumor differentiation, TNM stage, lymph node metastasis, with or without distant metastasis had statistical significance (P <0.05), uPA and Ki There was no significant difference in age, sex, tumor diameter and tumor location between the two groups (P> 0.05). The Spearman correlation analysis showed that the positive expression of uPA protein was negatively correlated with the degree of tumor differentiation (r = -0.333, P = 0.008), which was positively correlated with TNM staging and lymph node metastasis (r = 0.430, P <0.001; r = 0.400, P = 0.008), which was positively correlated with TNM stage, distant metastasis and lymph node metastasis (r = 0.494, P <0.001; r = 0.340, P = 0.006; r = 0.400, P = 0.001). Spearman correlation analysis showed that, The positive expression of uPA and Ki-67 protein in pancreatic cancer was positively correlated (r = 0.261, P = 0.032). Conclusion The positive expression of uPA and Ki-67 may be related to the invasion and metastasis of pancreatic cancer, which may be used as a reference index for the diagnosis, staging and prognosis of pancreatic cancer.
