目的 观察心肌背向散射积分 (integrated backscatter,IBS)参数在糖尿病患者中的变化 ,并探讨背向散射积分参数与心脏功能的关系。方法 采用 HP Sonos 5 5 0 0型超声波诊断仪 ,检测了 34例 型糖尿病患者及 1 6例正常人的心肌背向散射积分参数 (IBS、 IBS%、 CVIB、 CVIB%及 TGIB) ,并同时测量了心脏收缩功能指标 (EF)和心脏舒张功能指标 (E、 A、 E/ A、 EDT、 IVRT)。采样部位为胸骨旁左室长轴切面室间隔及左室后壁中间段。结果 发现糖尿病患者心肌背向散射积分参数与对照组间有显著性差异 ,IBS%及 TGIB增大 ,P<0 .0 0 1 ,而 CVIB及 CVIB%减小 ,P<0 .0 0 1 ,CVIB及 CVIB%与 EF间相关良好 ,IBS%、 CVIB、 CVIB%、 TGIB与舒张功能指标间密切相关。结论 糖尿病患者心肌组织特征与正常人明显不同 ,心肌背向散射积分参数与心脏功能指标相关良好 ,通过分析心肌背向散射积分参数不仅可以评价糖尿病患者的心肌病变 ,而且可以协助评价心脏功能
Objective To observe the changes of myocardial backscatter (IBS) parameters in diabetic patients and to explore the relationship between the parameters of backscatter integration and cardiac function. Methods The parameters of myocardial backscatter (IBS, IBS%, CVIB, CVIB% and TGIB) were measured in 34 patients with diabetes mellitus and 16 normal subjects by HP Sonos 5500 ultrasonic diagnostic apparatus. Simultaneous measurement Cardiac systolic function (EF) and diastolic function (E, A, E / A, EDT, IVRT) were measured. Sampling site for the parasternal left ventricular long axis of the interventricular septum and the middle of the posterior wall of the left ventricle. The results showed that there was a significant difference between the integral parameters of myocardial backscatter in diabetic patients and the control group, IBS% and TGIB increased, P <0.01, but CVIB and CVIB% decreased, P <0.01, CVIB and CVIB% had good correlation with EF, IBS%, CVIB, CVIB%, TGIB and diastolic function index were closely related. Conclusion The characteristics of myocardium in diabetics are obviously different from those in normal people. The integral parameters of myocardial backscatter are well correlated with cardiac function indexes. The analysis of myocardial backscatter integration parameters can not only evaluate the cardiomyopathy in diabetic patients, but also help to evaluate the cardiac function
