目的:了解膈食管膜解剖学及组织学结构和特点,探索膈食管膜在抗反流屏障中的作用。方法:对30例尸体下食管区域行解剖学观察,并作组织学切片,进行组织学观察。结果:膈食管膜由弹力纤维和胶原纤维组成,以弹力纤维为主。膈食管膜分上、下两层,分别起于膈上、下筋膜,上层向上,下层分层为上、下两叶后向上、下止于食管外膜,部分纤维插入纵肌束或环肌束间。较大胎龄胎尸、婴幼尸及成人尸在食管下段膈上2~4cm内左前侧均可见一纵行膈食管膜附着;而较小胎龄胎尸膈食管膜则呈伞状。结论:膈食管膜的解剖学及组织学特征提示:①膈食管膜随年龄增长而渐发育成形;②其在抗反流屏障中占有重要位置;③膈食管膜对发育中的食管可能起向下牵拉的作用;④膈食管膜发育不完善或变性可致抗反流作用减少
OBJECTIVE: To understand the anatomy and histology of the diaphragm esophageal membrane and to explore the role of the diaphragm esophageal membrane in the anti-reflux barrier. Methods: Anastomosis was performed on the lower esophagus in 30 cases and histological sections were made for histological observation. Results: The diaphragm esophageal membrane consists of elastic fibers and collagen fibers, mainly elastic fibers. Diaphragmatic esophageal membrane points, the next two layers, respectively, from the diaphragm, the lower fascia, the upper up, the lower stratification of the upper and lower leaves after up, the lower end of the esophageal adventitia, part of the longitudinal fibers inserted into the ring or ring Muscle bundle room. Larger gestational age cadaveric, baby corpse and adult corpse in the lower esophageal septum 2 ~ 4cm on the left front side can be seen a longitudinal diaphragmatic esophageal membrane attachment; and smaller gestational age phrenic diaphragm esophageal membrane was umbrella. Conclusion: The anatomy and histological features of the phrenic esophageal membrane suggest that: ① the phrenic esophageal membrane gradually shapes with age; ② it occupies an important position in the anti-reflux barrier; ③ the phrenic esophageal membrane may play an important role in the development of the esophagus The role of under the pull; ④ phrenic edema membrane imperfections or degeneration can cause anti-reflux effect decreased
