目的比较Dynamax和Twin-block两种功能矫治器治疗生长发育期安氏Ⅱ类下颌后缩患者的临床疗效。方法 28例生长发育期下颌后缩患者随机分为两组,分别应用Dynamax(D组,14例)和Twin-block(T组,14例)。测量治疗前、后X线头颅侧位片,并对相关的软硬组织测量指标及矫治时间等进行统计学处理。结果与治疗前比较,两组治疗后软组织面角均显著增加,面突角、上唇-审美平面距均显著减小(P<0.05),SNB角、Y轴角、下颌长、下颌平面角(MP-SN)、全面高、下面高均显著增加,ANB角均显著减小(P<0.05)。治疗后,D组下中切牙角(IMPA)显著增加(P<0.05),T组IMPA无显著增加。D组的治疗时间显著短于T组(P<0.05)。结论两种矫治器均能使下颌后缩患者得到较好的矫治。治疗后软组织侧貌都有极大的改善。Dynamax矫治器容易产生牙齿排列异常及前牙过度代偿等副作用,对下颌的前移效果略逊于Twin-block;但是治疗时间更短,患者更舒适。
Objective To compare the clinical efficacy of Dynamax and Twin-block two functional appliances in the treatment of patients with Class Ⅱ mandibular retrusion during growth and development. Methods Twenty - eight patients with mandibular retrusion during their growth and development were randomly divided into two groups: Dynamax (group D, 14 cases) and Twin - block (group T, 14 cases). Measurement before and after treatment of X-ray cephalospasm, and the relevant soft and hard tissue measurements and correction time for statistical analysis. Results Compared with those before treatment, the facial angles of soft tissue in both groups were significantly increased (P0.05), SNB angle, Y-axis angle, mandibular length and mandibular plane angle MP-SN). The total height and height of MP-SN increased significantly, while the ANB angle decreased significantly (P <0.05). After treatment, the IMPA of group D was significantly increased (P <0.05), while that of group T was no significant increase. The treatment time in group D was significantly shorter than that in group T (P <0.05). Conclusion Both orthodontic appliances can make the patients with mandibular retrusion better corrected. After treatment soft tissue profile has greatly improved. Dynamax appliance prone to abnormal tooth arrangement and anterior teeth overcompensation and other side effects, the effect of the mandibular advance slightly less than Twin-block; but shorter treatment times, patients are more comfortable.
