目的:探讨窒息新生儿尿乙酰-β-D氨基葡萄糖苷酶(NAG)、尿视黄醇结合蛋白(RBP)和β2微球蛋白(β2-MG)表达水平。方法:用对硝基苯酚比色法及ELISA测定窒息新生儿和正常新生儿生后第1、3、7天尿NAG活性和尿RBP、β2-MG含量。结果:窒息新儿生后第1天尿NAG,RBP和β2-MG水平均显著增高,与正常对照相比,差异有显著意义(P<0.01),且重度窒息足月儿、早产儿较轻度窒息足月儿、早产儿尿NAG,RBP和β2-MG水平更高,相互比较差异有显著意义(P<0.05或P<0.01)。窒息新儿生后第3天尿NAG,RBP水平仍较高,与正常对照相比,差异有显著意义(P<0.05及P<0.01);β2-MG含量显著下降,与正常对照相比,差异无显著意义(P>0.05)。窒息新儿生后第7天尿NAG活性、β2-MG含量均接近正常水平,与正常对照相比,差异无显著意义(P>0.05)。结论:窒息新生儿存在一定程度的肾功能损伤,其损伤程度似与尿NAG、RBP和β2-MG含量有关。随着窒息新生儿日龄的增加,尿NAG,RBP和β2-MG水平逐步趋于正常。
Objective: To investigate the expression of urinary acetyl-β-D-glucosaminidase (NAG), urinal retinol binding protein (RBP) and β2 microglobulin (β2-MG) in asphyxia neonates. Methods: The urinary NAG activity and urinary RBP, β2-MG contents of neonatal asphyxia neonates and normal neonates were detected by p-nitrophenol colorimetry and ELISA. Results: Compared with normal control, the levels of urinary NAG, RBP and β2-MG were significantly increased on the first day after asphyxia (P <0.01), and severe asphyxia term infants and premature infants were lighter Degree of asphyxia term infants, preterm children urinary NAG, RBP and β2-MG levels were higher, the difference was significant (P <0.05 or P <0.01). The levels of urinary NAG and RBP were still higher on the 3rd day after asphyxia in newborns, which were significantly different from the normal control (P <0.05 and P <0.01), while the content of β2-MG was significantly decreased. Compared with the normal controls, There was no significant difference (P> 0.05). Urinary NAG activity and β2-MG content of neonatal asphyxia newborn was close to the normal level on the 7th day, which had no significant difference compared with the normal control (P> 0.05). CONCLUSIONS: There is a certain degree of renal dysfunction in neonates with asphyxia, and the degree of damage seems to be related to the contents of urinary NAG, RBP and β2-MG. Urine NAG, RBP and β2-MG levels tended to be normal with the increasing of neonatal asphyxial days.
