目的了解Robinsoniella peoriensis(R.peoriensis)临床分离菌株的形态特点、生化特征及耐药情况,为深入研究该菌提供资料。方法在对四川大学华西医院ICU医疗单元患者进行肛拭子筛查时,从一例36岁的男性肺癌患者身上分离到了一株R.peoriensis,采用VitekⅡ全自动微生物鉴定及药敏分析系统(BioMérieux,Marcy-I′toile,France)的厌氧菌鉴定卡进行生化鉴定,琼脂稀释法体外药敏试验了解该菌耐药情况。结果生化鉴定示R.peoriensis对蔗糖、β-半乳糖吡喃苷酶吲哚酚、阿拉伯糖苷酶等反应阳性,而对鉴定卡的其他成分反应阴性。体外药敏试验证实该菌对克林霉素、利福平、莫西沙星耐药,而对万古霉素、甲硝唑、四环素敏感。结论 R.peoriensis是一种革兰阳性的厌氧芽孢杆菌,对蔗糖等多种物质反应阳性,既可导致感染又可引起定植。本研究中的R.peoriensis临床分离菌株对克林霉素、利福平、莫西沙星耐药,并且克林霉素和利福平最低抑菌浓度均大于512μg/mL。
Objective To understand the morphological characteristics, biochemical characteristics and drug resistance of clinical isolates of Robinsoniella peoriensis (R. peoriensis) and provide information for further study on the bacteria. Methods In the screening of anal swabs in ICU medical units of West China Hospital of Sichuan University, a R.peoriensis strain was isolated from a 36-year-old male patient with lung cancer using Vitek Ⅱ automatic microbial identification and drug sensitivity analysis system (BioMérieux, Marcy-I’toile, France) biochemical identification of anaerobic identification card, agar dilution method in vitro susceptibility testing to understand the bacterial resistance. Results Biochemical identification showed that R. pecoriensis was positive for sucrose, β-galactopyranosidase, indoxylol and arabinosidase, but negative for the other components of the identification card. In vitro susceptibility test confirmed that the bacteria clindamycin, rifampicin, moxifloxacin resistance, and vancomycin, metronidazole, tetracycline sensitive. Conclusions R. peoriensis is a Gram-positive anaerobic bacillus that is positive for a variety of substances, such as sucrose, and can cause both infection and colonization. In this study, R.peoriensis clinical isolates were resistant to clindamycin, rifampicin and moxifloxacin, and the minimum inhibitory concentrations of clindamycin and rifampicin were both greater than 512μg / mL.
