本文报告了纽约MSK癌症中心1979.3-1979.4间收治的44例小细胞肺癌用化疗、全脑予防性照射、原发灶及纵膈高剂量照射综合治疗的结果,并回答了下列问题:①本治疗方案能否提高完全缓解率和存活率;②能否安全地高剂量照射纵膈;③缩短全脑预防照射过程是否安全有效,价值如何? 材料方法:全部均经组织学或细胞学证实,即往未经治疗并未患过其它恶性病。局部病变者24例,广泛病变者占20例。治疗计划见图示。在初步化疗后2个月开始低剂量化疗及放疗。全脑预防照射用~(60)Co平行对穿野,剂量为500rad×3,休息4天,再给300rad×5,或相当于4000rad之NSD值。原发灶及纵膈用10MV直线加速器多野照射,250rad×18,4次/周,总量5000rad。照射的第1、15天给予CTX及VCR化疗。结果:24例局部病变中化疗后16例完全缓解,
This article reports the results of a combination of chemotherapy, total brain prophylaxis, primary foci, and high dose radiation therapy for 44 patients with small cell lung cancer treated between 1979.3-1979.4 at the MSK Cancer Center in New York City, and answered the following questions: 1 treatment Whether the program can improve the complete remission rate and survival rate; 2 whether it can safely irradiate the mediastinum; 3 whether shortening the whole brain to prevent irradiation process is safe and effective, and how is the value? Materials and methods: all confirmed by histology or cytology, namely Untreated and not suffering from other malignant diseases. There were 24 patients with local lesions and 20 patients with extensive lesions. Treatment plan see illustration. Low-dose chemotherapy and radiotherapy started 2 months after the initial chemotherapy. The whole brain was irradiated with ~(60)Co parallel to the field, the dose was 500 rad×3, rested for 4 days, and gave 300 rad×5, or an NSD equivalent to 4000 rad. Primary tumors and mediastinals were irradiated with multiple fields using a 10 MV linear accelerator, 250 rad×18, 4 times/week, for a total of 5000 rad. CTX and VCR chemotherapy were given on days 1 and 15 of irradiation. Results: In the 24 cases of local lesions, 16 cases were completely relieved after chemotherapy.
