目的:探讨依那普利联合利尿剂治疗高血压危象的临床疗效。方法:我院2011年7月~2013年7月共收治91例高血压危象患者,按照随机分层分组法将91例患者分为观察组48例和对照组43例,对照组给予静脉注射乌拉地尔,观察组在此基础上口服依那普利和小剂量氢氯噻嗪,观察两组治疗前后血压、心率变化,同时观察临床总有效率和不良反应发生率。结果:两组治疗后血压和心率相比治疗前显著下降(P<0.05),两组降压起效时间、持续时间和总有效率无显著差异(P>0.05)。观察组1h内降压幅度、心率反射性增高情况及不良反应发生率均显著优于对照组,两组间差异有统计学意义(P<0.05)。结论:依那普利联合小剂量利尿剂能显著降低高血压危象患者的血压,在持续稳定降低血压同时,不会反射性增加心率和不良反应发生率,具有较高的临床应用价值。
Objective: To investigate the clinical efficacy of enalapril plus diuretic in the treatment of hypertensive crisis. Methods: From July 2011 to July 2013, 91 cases of hypertensive crisis were treated in our hospital. According to the randomized stratified grouping method, 91 patients were divided into observation group (48 cases) and control group (43 cases). The control group received intravenous injection Urapidil, the observation group on this basis, oral enalapril and low-dose hydrochlorothiazide, observed two groups before and after treatment of blood pressure, heart rate changes, while observing the clinical total efficiency and adverse reaction rates. Results: After treatment, blood pressure and heart rate decreased significantly (P <0.05), but there was no significant difference between the two groups in the onset time, duration and total effective rate (P> 0.05). The observation group 1h blood pressure range, heart rate reflex increase and the incidence of adverse reactions were significantly better than the control group, the difference between the two groups was statistically significant (P <0.05). Conclusion: Enalapril combined with low-dose diuretic can significantly reduce the blood pressure in patients with hypertensive crisis. It will not increase the heart rate and incidence of adverse reactions during the sustained and stable lowering of blood pressure, and has high clinical value.
