目的分析白血病患者假性低血糖的特点及其发生原因。方法 2007年至2010年收治白血病患者57例,均排除糖尿病,且无低血糖临床表现。采用罗氏血糖仪测定其即时血糖,用葡萄糖氧化酶法和已糖激酶法测定标本水浴1/2h、1h、2h后的血糖浓度,同时测定乳酸浓度。结果两法检测的血糖浓度有相关性(r=0.995,P<0.01);标本即时血糖在正常范围,水浴后不同时段血糖浓度明显低于正常且随时间延长递降,乳酸递升(P均<0.01);47例治疗后白血病病情好转患者血标本水浴2h检测较治疗前标本水浴2h的葡萄糖浓度升高(达正常值),乳酸浓度下降(P均<0.01)。结论白血病患者假性低血糖患者的血糖在体内并不低,而是标本离体后出现。某些类型的白血病细胞异常增生,可能对葡萄糖的酵解能力增强,导致白血病患者假性低血糖。
Objective To analyze the characteristics and cause of hypoglycemia in patients with leukemia. Methods Fifty-seven patients with leukemia were enrolled from 2007 to 2010, all of whom excluded diabetes and had no clinical manifestations of hypoglycaemia. The blood glucose level was determined by Roche blood glucose meter. The blood glucose level was determined by glucose oxidase method and hexokinase method after 1 / 2h, 1h, 2h, respectively. Meanwhile, the concentration of lactate was determined. Results The blood glucose levels of the two methods were correlated (r = 0.995, P <0.01). The real-time blood glucose of the samples was in the normal range. The blood glucose levels of the samples were significantly lower than normal and descended with the lapse of time ); 47 cases of patients with leukemia after treatment improved blood samples 2h detection of water samples before treatment than the water bath 2h glucose concentration (up to normal), lactate concentration decreased (P all <0.01). Conclusion The blood sugar of patients with hypoglycemia in patients with leukemia is not low in the body, but the specimens appear after ex vivo. Abnormal proliferation of certain types of leukemia cells, may increase the ability of glucose glycolysis, leading to hypoglycemia in patients with leukemia.
